玻璃体腔内注射康柏西普治疗视网膜静脉阻塞继发黄斑水肿临床研究

Clinical Study of Intravitreal Injection of Compazepine for Retinal Vein Obstruction Secondary to Macular Edema

摘要 目的探讨玻璃体腔内注射康柏西普在视网膜静脉阻塞(RVO)继发黄斑水肿(ME)患者中的应用价值.方法选择2020年3月—2021年12月该院收治的RVO继发ME患者84例(84眼),按随机数字表法分为两组.对照组42例(42眼)采用玻璃体腔内注射曲安奈德治疗,研究组42例(42眼)采用玻璃体腔内注射康柏西普治疗,均治疗6个月.对比分析两组中心视网膜厚度、最佳矫正视力、血管内皮生长因子(VEGF)水平、并发症发生情况.结果治疗后,两组中心视网膜厚度薄于治疗前、最佳矫正视力高于治疗前、VEGF水平低于治疗前,且研究组VEGF水平低于对照组,组间差异有统计学意义(P<0.05),但两组中心视网膜厚度、最佳矫正视力相比,组间差异无统计学意义(P>0.05).研究组并发症发生率4.76%低于对照组的19.05%,差异有统计学意义(P<0.05).结论玻璃体腔内注射康柏西普和注射曲安奈德均能够降低RVO继发ME患者的中心视网膜厚度,提高患眼视力,但康柏西普更利于降低VEGF水平,且并发症较少,临床应用价值更高. Objective To investigate the value of intravitreal injection of Compazep in patients with retinal vein occlusion(RVO)secondary to macular edema(ME).Methods 84 patients(84 eyes)with RVO secondary to ME admitted to the hospital from March 2020 to December 2021 were selected and divided into two groups according to the random number table method.42 cases(42 eyes)in the control group were treated with intravitreal injection of tretinoin and 42 cases(42 eyes)in the study group were treated with intravitreal injection of Compazine,both for 6 months.The central retinal thickness,best corrected visual acuity,vascular endothelial growth factor(VEGF)level,and complications in the two groups were compared and analyzed.Results After treatment,the central retinal thickness in both groups was thinner than before treatment,the best corrected visual acuity was higher than before treatment,and the VEGF level was lower than before treatment,and the VEGF level in the study group was lower than that in the control group,and the differences between the groups were statistically significant(P<0.05),but there were no statistically significant differences between the groups compared with the central retinal thickness and best corrected visual acuity in both groups(P>0.05).The complication rate of 4.76%in the study group was lower than 19.05%of in the control group,and the difference was statistically significant(P<0.05).Conclusion Both intravitreal injection of Compazepine and tretinoin were able to reduce the central retinal thickness and improve the visual acuity of the affected eyes in patients with RVO secondary to ME,but Compazepine was more beneficial in reducing VEGF levels and had fewer complications,making it more valuable for clinical application.
