腹腔镜卵巢良性囊肿剥除术中双极电凝止血对患者手术相关指标及卵巢储备功能的影响

Effect of Bipolar Electrocoagulation Hemostasis on Operation Related Indexes and Ovarian Reserve Function in Patients during Laparoscopic Benign Ovarian Cyst Stripping

目的分析腹腔镜卵巢良性囊肿剥除术中双极电凝止血对患者手术相关指标及卵巢储备功能的影响.方法选取2019年8月—2021年8月该院诊治的84例卵巢良性囊肿患者为研究对象,按随机数字表法分为两组,每组42例.对照组于腹腔镜卵巢良性囊肿剥除术中应用缝合止血,观察组于术中应用双极电凝止血,观察至术后1个月.对比两组手术相关指标、术前和术后1个月的卵巢储备功能.结果观察组手术时间短于对照组,术中出血量少于对照组,组间差异有统计学意义(P<0.05).术前,两组抗苗勒氏管激素(AMH)、窦卵泡计数(AFC)、卵泡刺激素(FSH)、雌二醇(E_(2))、促黄体生成素(LH)水平相比,组间差异无统计学意义(P>0.05);术后1个月,观察组AMH水平低于对照组,AFC少于对照组,FSH、E_(2)、LH水平均高于对照组,组间差异有统计学意义(P<0.05).结论腹腔镜卵巢良性囊肿剥除术中采用双极电凝止血可缩短手术时间,减少术中出血量,但对卵巢储备功能影响较大.

Objective To analyze the effect of bipolar electrocoagulation hemostasis on operation related indexes and ovarian reserve function in patients during laparoscopic benign ovarian cyst stripping.Methods 84 patients with benign ovarian cysts diagnosed and treated in the hospital from August 2019 to August 2021 were selected as the study subjects.They were divided into two groups according to the random number table method,with 42 cases in each group.The control group used suture hemostasis during laparoscopic benign ovarian cyst stripping,and the observation group used bipolar electrocoagulation hemostasis during the operation.The observation period was 1 month after the operation.The operation related indexes and ovarian reserve function before and 1 month after operation were compared between the two groups.Results The operation time of the observation group was shorter than that of the control group,and the intraoperative bleeding volume was less than that of the control group,and the differences between the groups were statistically significant(P<0.05).Before operation,there were no significant differences in the levels of anti Mullerian hormone(AMH),antral follicle count(AFC),follicle stimulating hormone(FSH),estradiol(E_(2))and luteinizing hormone(LH)between the two groups(P>0.05);1 month after operation,the level of AMH in the observation group was lower than that in the control group,AFC was less than that in the control group,and the levels of FSH,E_(2)and LH were higher than those in the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion The bipolar electrocoagulation can shorten the operation time and reduce the intraoperative bleeding volume during laparoscopic benign ovarian cyst stripping,but it has a great impact on ovarian reserve function.
