针灸联合脊柱定点旋转复位手法应用于腰椎间盘突出症患者的临床效果

Clinical Effect of Acupuncture Combined with Fixed-Point Rotation Reduction Manipulation of Spine in Patients with Lumbar Disc Herniation

目的探讨针灸联合脊柱定点旋转复位手法治疗腰椎间盘突出症(LDH)的临床效果.方法选取2020年4月—2022年2月我院收治的86例LDH患者,按随机数字表法分为两组,各43例.对照组予以常规西药治疗,观察组在对照组基础上加用针灸及脊柱定点旋转复位手法治疗,持续治疗4周.比较两组临床疗效、治疗前和治疗4周后的腰椎功能、腰背疼痛程度、日常生活能力、炎症反应及不良反应.结果观察组治疗总有效率95.35%(41/43)高于对照组,治疗4周后日本骨科协会评估治疗分数量表评分(25.12±2.24)分、Bathel指数评定量表评分(83.69±7.12)分均高于对照组的(23.34±2.06)分、(76.52±6.87)分,视觉模拟评分(1.89±0.22)分低于对照组的(2.35±0.28)分,组间差异有统计学意义(P<0.05);观察组治疗4周后C反应蛋白水平(6.05±1.12)mg/L、肿瘤坏死因子-α水平(45.33±4.14)pg/mL、白细胞介素-6水平(16.25±1.39)pg/mL均低于对照组的(7.89±1.16)mg/L、(51.62±4.26)pg/mL、(18.79±1.45)pg/mL,组间差异有统计学意义(P<0.05);两组均无严重不良反应发生.结论针灸联合脊柱定点旋转复位手法可减轻LDH患者腰部炎症反应及疼痛程度,加快腰椎功能恢复,增强日常生活能力,安全可靠.

Objective To explore the clinical effect of acupuncture combined with fixed-point rotation reduction manipulation of spine in the treatment of lumbar disc herniation(LDH).Methods 86 patients with LDH admitted to our hospital from April 2020 to February 2022 were selected and divided into two groups according to the random number table method,with 43 cases in each group.The control group was treated with conventional western medicine,and the observation group was treated with acupuncture and fixed-point rotation reduction manipulation of spine on the basis of the control group,with both interventions for 4 weeks.The clinical efficacy,lumbar function,lumbar and back pain degree,daily living ability,inflammatory reaction before and after 4 weeks of treatment and adverse reactions were compared between the two groups.Results The total effective rate of treatment 95.35%(41/43)of the observation group was higher than that of the control group,the Japanese Orthopaedic Association scores scale score(25.12±2.24)points and the Barthel index score(83.69±7.12)points after 4 weeks of treatment were higher than(23.34±2.06)points and(76.52±6.87)points of the control group,and the visual analogue scale score(1.89±0.22)points was lower than(2.35±0.28)points of the control group,the differences between the groups were statistically significant(P<0.05);C-reactive protein level(6.05±1.12)mg/L,tumor necrosis factor-αlevel(45.33±4.14)pg/mL and interleukin-6 level(16.25±1.39)pg/mL after 4 weeks of treatment in the observation group were lower than(7.89±1.16)mg/L,(51.62±4.26)pg/mL and(18.79±1.45)pg/mL in the control group,the differences between the groups were statistically significant(P<0.05);there were no serious adverse reactions in both groups.Conclusion The acupuncture combined with fixed-point rotation reduction manipulation of spine can reduce the inflammatory reaction and pain degree in the lumbar region of patients with LDH,accelerate the recovery of lumbar function,and enhance the ability of daily living.It is safe and reliable.
