凝血酶冻干粉联合奥美拉唑对A1级消化性溃疡出血患者凝血功能的影响

Effect of Thrombin Lyophilized Powder Combined with Omeprazole on Coagulation Function in Patients with Bleeding from Grade A1 Peptic Ulcer

目的探讨凝血酶冻干粉联合奥美拉唑对A1级消化性溃疡出血患者凝血功能的影响.方法选择2020年2月-2022年1月该院收治的93例A1级消化性溃疡出血患者作为研究对象,按照随机数字表法分为对照组46例与观察组47例.对照组给予奥美拉唑治疗,观察组在对照组基础上联合凝血酶冻干粉治疗.比较两组止血效果、治疗前和随访1个月后凝血功能以及胃肠激素水平.结果观察组总止血率93.62%高于对照组的78.26%,差异有统计学意义(P<0.05);随访1个月后,观察组凝血酶原时间、活化部分凝血活酶时间均短于对照组,纤维蛋白原、血小板比容均高于对照组,平均血小板体积小于对照组,组间差异有统计学意义(P<0.05);随访1个月后,观察组胃动素、胃泌素水平均高于对照组,组间差异有统计学意义(P<0.05).结论凝血酶冻干粉联合奥美拉唑应用于A1级消化性溃疡出血患者,有利于调节血小板参数,改善凝血功能,调整相关胃肠激素紊乱,具有良好的止血效果.

Objective To investigate the effect of thrombin lyophilized powder combined with omeprazole on coagulation function in patients with bleeding from grade A1 peptic ulcer.Methods 93 patients with bleeding from grade A1 peptic ulcer admitted to the hospital from February 2020 to January 2022 were selected as the study subjects,and were divided into a control group of 46 cases and an observation group of 47 cases according to the random number table method.The control group was treated with omeprazole,and the observation group was treated with thrombin lyophilized powder on the basis of the control group.The hemostatic effect,coagulation function and gastrointestinal hormone level before treatment and after 1 month follow-up were compared between the two groups.Results The total hemostatic rate 93.62%of the observation group was higher than 78.26%of the control group,and the difference was statistically significant(P<0.05);after 1 month follow-up,prothrombin time and activated partial thromboplastin time in the observation group were shorter than those in the control group,fibrinogen and platelet hematocrit were higher than those in the control group,and mean platelet volume was smaller than that in the control group,and the differences between the groups were statistically significant(P<0.05);after 1 month follow-up,the levels of motilin and gastrin in the observation group were higher than those in the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion The application of thrombin lyophilized powder combined with omeprazole in patients with bleeding from grade A1 peptic ulcer is beneficial to regulating platelet parameters,improving coagulation function,and regulating gastrointestinal hormone disorders.It has a good hemostatic effect.
