中医针灸联合甲钴胺片治疗糖尿病周围神经病变的临床疗效

Clinical Efficacy of Traditional Chinese Medicine Acupuncture Combined with Mecobalamin Tablets in the Treatment of Diabetic Peripheral Neuropathy

目的观察中医针灸联合甲钴胺片治疗糖尿病周围神经病变(DPN)的效果.方法选取2019年6月—2021年6月本院收治的84例DPN患者,按随机数字表法分为两组,每组42例.对照组采用甲钴胺片治疗,观察组在此基础上采用中医针灸治疗,均连续治疗4周.对比两组的临床疗效、中医证候积分、血液流变学指标、神经传导速度及不良反应发生情况.结果观察组的治疗总有效率为95.24%,高于对照组的80.95%,差异有统计学意义(P<0.05).治疗后,观察组的肢体麻木、疼痛、精神不振、乏力积分及全血高切粘度、红细胞压积、全血低切粘度水平均低于对照组,正中神经、腓总神经的感觉神经传导速度及运动神经传导速度均快于对照组,组间差异有统计学意义(P<0.05).观察组的不良反应发生率为4.76%,与对照组的9.52%比较,差异无统计学意义(P>0.05).结论中医针灸联合甲钴胺片治疗DPN患者的效果显著,能够缓解其各项症状,调节血液流变学指标,加快神经传导速度,且无严重不良反应,值得临床推广使用.

Objective To observe the effect of traditional Chinese medicine acupuncture combined with mecobalamin tablets on diabetic peripheral neuropathy(DPN).Methods A total of 84 DPN patients admitted to our hospital from June 2019 to June 2021 were selected and were divided into two groups according to the random number table method,with 42 cases in each group.The control group was treated with mecobalamin tablets,and the observation group was treated with traditional Chinese medicine acupuncture on this basis.Both groups were treated for 4 weeks.The clinical efficacy,TCM syndrome scores,hemorheology indexes,nerve conduction velocity and adverse reactions were compared between the two groups.Results The total effective rate of the observation group was 95.24%,which was higher than 80.95%of the control group,and the difference was statistically significant(P<0.05).After treatment,the numbness,pain,depressed,fatigue scores and the levels of whole blood high shear viscosity,erythrocyte deposited,whole blood low shear viscosity of the observation group were lower than those of the control group,and the sensory nerve conduction velocity and motor nerve conduction velocity of median nerve and common peroneal nerve in the observation group were faster than those in the control group,the differences between the groups were statistically significant(P<0.05).The incidence of adverse reactions in the observation group was 4.76%,compared with 9.52%in the control group,and the difference was not statistically significant(P>0.05).Conclusion Traditional Chinese medicine acupuncture combined with mecobalamin tablets has a significant effect in the treatment of DPN patients,which can relieve the symptoms,regulate the hemorheology indexes,accelerate nerve conduction velocity,and has no serious adverse reactions.It is worthy of clinical application.
