α-硫辛酸联合甲钴胺对2型糖尿病周围神经病变患者炎症反应及氧化应激指标的影响 被引量:1

Effects of α-Lipoic Acid Combined with Mecobalamin on Inflammatory Response and Oxidative Stress Indicators in Patients with Type 2 Diabetes Peripheral Neuropathy

摘要 目的探讨α-硫辛酸联合甲钴胺在2型糖尿病周围神经病变(DPN)患者中的疗效.方法选择该院2020年6月—2021年5月收治的88例2型DPN患者,按随机数字表法分为两组.对照组44例采用甲钴胺进行治疗,观察组44例在对照组基础上采用α-硫辛酸进行治疗,两组均持续治疗2周.对比两组临床疗效、超敏C反应蛋白(hs-CRP)及肿瘤坏死因子-α(TNF-α)、硝基络氨酸(NT)、超氧化物歧化酶(SOD)、丙二醛(MDA)及不良反应.结果观察组临床总有效率95.45%高于对照组的81.82%,差异有统计学意义(P<0.05);观察组治疗2周后hs-CRP水平(4.46±1.07)mg/L、TNF-α水平(17.47±2.77)mmol/L均低于对照组的(6.38±1.35)mg/L、(26.37±3.05)mmol/L,组间差异有统计学意义(P<0.05);观察组治疗2周后SOD水平(80.60±14.22)μU/L高于对照组的(70.76±12.75)μU/L,NT水平(18.24±4.67)U/L、MDA水平(4.77±1.05)μmol/L均低于对照组的(25.26±5.05)U/L、(6.25±1.23)μmol/L,组间差异有统计学意义(P<0.05);两组不良反应发生率相比,差异无统计学意义(P>0.05).结论α-硫辛酸联合甲钴胺治疗2型DPN的疗效确切,可减轻炎性反应与氧化应激损伤,且不会增加不良反应的发生. Objective To explore efficacy ofα-lipoic acid combined with mecobalamin in patients with type 2 diabetes peripheral neuropathy(DPN).Methods 88 patients with type 2 DPN admitted to the hospital from June 2020 to May 2021 were selected and divided into two groups according to the random number table method.The control group of 44 cases was treated with mecobalamin,while the observation group of 44 cases was treated withα-lipoic acid on the basis of the control group,and both groups were treated for 2 weeks.The clinical efficacy,hypersensitive C-reactive protein(hs-CRP)and tumor necrosis factor-α(TNF-α),nitrotyrosine(NT),superoxide dismutase(SOD),malondialdehyde(MDA)and adverse reactions were compared between the two groups.Results The total clinical effective rate 95.45%of the observation group was higher than 81.82%of the control group,the difference was statistically significant(P<0.05);the hs-CRP level(4.46±1.07)mg/L and TNF-αlevel(17.47±2.77)mmol/L in the observation group after 2 weeks of treatment were lower than(6.38±1.35)mg/L,(26.37±3.05)mmol/L in the control group,and the differences between the groups were statistically significant(P<0.05);the SOD level(80.60±14.22)μU/L in the observation group after 2 weeks of treatment was higher than(70.76±12.75)μU/L in the control group,NT level(18.24±4.67)U/L,MDA level(4.77±1.05)μmol/L were lower than(25.26±5.05)U/L、(6.25±1.23)μmol/L in the control group,the differences between the groups were statistically significant(P<0.05);there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Theα-lipoic acid combined with mecobalamin is effective in the treatment of type 2 DPN,which can reduce inflammatory reaction and oxidative stress injury without increasing the occurrence of adverse reactions.
