Effect of High-Flow Nasal Cannula Oxygen Therapy Combined with Human Immunoglobulin on Immune Function and Inflammatory Response of Neonatal Pneumonia

摘要 目的研究经鼻高流量鼻导管湿化氧疗(HFNC)结合人免疫球蛋白治疗新生儿肺炎对免疫功能及炎症反应的影响.方法选择2019年1月—2021年12月本院收治的108例新生儿肺炎患儿,按随机原则分为两组,每组54例.对照组采用经鼻高流量鼻导管湿化氧疗,研究组采用经鼻高流量鼻导管湿化氧疗结合人免疫球蛋白治疗.对比两组患儿治疗7 d后的临床疗效、免疫球蛋白A(IgA)、免疫球蛋白M(IgM)、免疫球蛋白G(IgG)及高敏C反应蛋白(hs-CRP)、降钙素原(PCT)、白细胞介素-6(IL-6).结果研究组的治疗总有效率为94.44%,高于对照组的81.48%,差异有统计学意义(P<0.05).治疗后,两组患儿的血清IgA、IgM、IgG水平均升高,且研究组各指标水平均高于对照组,组间差异有统计学意义(P<0.05).治疗后,两组患儿的血清hs-CRP、PCT、IL-6水平均降低,且研究组各指标水平均低于对照组,组间差异有统计学意义(P<0.05).结论经鼻高流量鼻导管湿化氧疗结合人免疫球蛋白治疗新生儿肺炎的疗效确切,可改善患儿症状及体征,增强机体免疫功能,减轻炎症反应,值得临床推广使用. Objective To study the effect of high-flow nasal cannula oxygen therapy(HFNC)combined with human immunoglobulin on immune function and inflammatory response in neonates with pneumonia.Methods The A total of 108 children with neonatal pneumonia admitted to our hospital from January 2019 to December 2021 were randomly divided into two groups,with 54 cases in each group.The control group received HFNC oxygen therapy,and the study group received HFNC oxygen therapy combined with human immunoglobulin.The clinical efficacy,immunoglobulin A(IgA),immunoglobulin M(IgM),immunoglobulin G(IgG)and high sensitivity C-reactive protein(hs-CRP),procalcitonin(PCT),interleukin-6(IL-6)were compared between the two groups after 7 days of treatment.Results The total effective rate of the study group was 94.44%,which was higher than 81.48%of the control group,the difference was statistically significant(P<0.05).After treatment,the levels of serum IgA,IgM and IgG in the two groups were increased,and the levels of all indexes in the study group were higher than those in the control group,the differences between the groups were statistically significant(P<0.05).After treatment,the levels of serum hS-CRP,PCT and IL-6 in the two groups were decreased,and the levels of all indicators in the study group were lower than those in the control group,the differences between the groups were statistically significant(P<0.05).Conclusion HFNC oxygen therapy combined with human immunoglobulin is effective in the treatment of neonatal pneumonia,which can improve the symptoms and signs of children,enhance the immune function of the body,reduce inflammatory response,and is worthy of clinical application.
