早期四级活动联合气压治疗预防轻型颅脑损伤下肢深静脉血栓形成的临床效果

Clinical Effect of Early Four-Level Activity Combined with Barometric Therapy on the Prevention of Lower Extremity Deep Venous Thrombosis in Patients with Mild Craniocerebral Injury

摘要 目的探究早期四级活动联合气压治疗预防轻型颅脑损伤下肢深静脉血栓(DVT)形成的临床效果.方法选择2021年5月—2022年2月本院收治的82例轻型颅脑损伤患者为研究对象,按随机数字表法分为对照组(n=41)和观察组(n=41).对照组进行常规干预,观察组采取早期四级活动联合气压治疗,均持续干预1个月.比较两组DVT发生情况、干预前后凝血功能、血液流速、血液流变学.结果观察组DVT发生率7.32%低于对照组的29.27%,差异有统计学意义(P<0.05).两组干预前凝血功能、血液流速、血液流变学指标对比,组间差异无统计学意义(P>0.05);观察组干预后凝血酶原时间(13.83±1.15)s、活化部分凝血活酶时间(36.15±3.42)s均长于对照组,D-二聚体水平(377.28±15.26)μg/L低于对照组,股静脉平均血液流速(31.46±3.39)cm/s、腘静脉平均血液流速(21.79±2.46)cm/s均快于对照组,全血低切黏度(37.22±4.13)mPa·s、全血高切黏度(5.10±0.65)mPa·s、红细胞聚集指数(4.18±0.53)均低于对照组,组间差异有统计学意义(P<0.05).结论早期四级活动联合气压治疗能够有效改善轻型颅脑损伤患者血液流变学及凝血功能,加快血液流速,减少DVT的发生. Objective To explore the clinical effect of early four-level activity combined with barometric therapy in preventing the lower extremity deep vein thrombosis(DVT)in mild craniocerebral injury.Methods A total of 82 patients with mild craniocerebral injury who were admitted to the hospital from May 2021 to February 2022 were selected as the research objects and divided into a control group(n=41)and an observation group(n=41)according to random number table method.The control group received routine intervention,while the observation group received early four-level activities combined with barometric therapy,and the interventions continued for 1 month.The incidence of DVT,coagulation function,blood flow rate and hemorheology before and after intervention were compared between the two groups.Results The incidence of DVT 7.32%in the observation group was lower than 29.27%in the control group,and the difference was statistically significant(P<0.05).There were no significant differences in coagulation function,blood flow rate and hemorheology indexes between the two groups before intervention(P>0.05);after intervention,prothrombin time(13.83±1.15)s and activated partial thromboplastin time(36.15±3.42)s in the observation group were longer than those in the control group,and D-dimer level(377.28±15.26)μg/L was lower than that in the control group,the average femoral vein blood velocity(31.46±3.39)cm/s and average popliteal vein blood velocity(21.79±2.46)cm/s were faster than those of the control group,the whole blood low-shear viscosity(37.22±4.13)mPa·s,the whole blood high-shear viscosity(5.10±0.65)mPa·s and the erythrocyte aggregation index(4.18±0.53)were lower than those of the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion Early four-level activity combined with barometric therapy can effectively improve hemorheology and coagulation function,accelerate blood flow,and reduce the incidence of DVT in patients with mild craniocerebral injury.
