体外冲击波结合康复训练对膝关节术后功能障碍的临床效果分析

Analysis of Clinical Effect of Extracorporeal Shock Wave Combined with Rehabilitation Training on Postoperative Functional Disability of Knee Joint

摘要 目的探讨对膝关节术后功能障碍患者采用体外冲击波结合康复训练的临床效果.方法选择该院2021年1月—2022年4月收治的62例膝关节术后功能障碍患者,以随机数表法分为两组.对照组(n=31)采用常规康复训练,观察组(n=31)在对照组基础上采用体外冲击波干预.比较两组关节活动度优良率、疼痛视觉模拟法(VAS)评分及美国纽约特种外科医院(HSS)膝关节评分.结果观察组关节活动度优良率为95.65%,高于对照组的80.43%,差异有统计学意义(P<0.05).干预后,观察组静息、运动时VAS评分分别为(1.42±0.34)分、(1.50±0.45)分,均高于对照组,组间差异有统计学意义(P<0.05).干预后,观察组HSS膝关节评分中疼痛、功能、活动度、肌力、屈曲畸形、稳定性分别为(26.04±3.27)分、(18.52±2.51)分、(16.21±1.42)分、(8.26±0.95)分、(8.18±1.05)分、(8.62±0.74)分,均高于对照组,组间差异有统计学意义(P<0.05).结论对膝关节术后功能障碍患者采用体外冲击波结合康复训练能提高其关节活动度,减轻疼痛程度,改善关节功能. Objective To explore the clinical effect of extracorporeal shock wave combined with rehabilitation training in patients with postoperative functional disability of the knee joint.Methods A total of 62 patients with postoperative functional disability of knee joint who were admitted to our hospital from January 2021 to April 2022 were selected and divided into two groups by random number table method.The control group(n=31)received conventional rehabilitation training,and the observation group(n=31)received extracorporeal shock wave intervention on the basis of the control group.The excellent and good rate of joint range of motion,the visual analogue scale(VAS)score and the Hospital for Special Surgery(HSS)knee score were compared between the two groups.Results The excellent and good rate of joint range of motion in the observation group was 95.65%,which was higher than 80.43%in the control group,and the difference was statistically significant(P<0.05).After the intervention,the VAS scores of the observation group at rest and during exercise were(1.42±0.34)points and(1.50±0.45)points,respectively,which were higher than those in the control group,and the differences between the groups were statistically significant(P<0.05).After the intervention,the HSS knee scores of pain,function,mobility,muscle strength,flexion deformity,and stability in the observation group were(26.04±3.27)points,(18.52±2.51)points,(16.21±1.42)points,(8.26±0.95)points,(8.18±1.05)points,and(8.62±0.74)points,respectively,which were higher than those in the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion The use of extracorporeal shock wave combined with rehabilitation training in patients with postoperative functional disability of knee joint surgery can improve their joint range of motion,reduce pain,and improve joint function.
