齐刺阿是穴配合温针灸治疗肩周炎80例疗效观察

Clinical Observation of 80 Cases of Scapulohumeral Periarthritis Treated by Triple Puncture Ashi Point Combined with Warm Acupuncture

目的探讨肩周炎患者采用齐刺阿是穴配合温针灸治疗的临床效果.方法选择本院2019年1月—2022年1月收治的80例肩周炎患者,采用随机数表法分组,对照组(n=40)采用常规针刺治疗,观察组(n=40)在此基础上采用齐刺阿是穴配合温针灸治疗.比较两组的临床疗效,采用Constant-Murley量表评价肩关节功能,另检测治疗前后的肿瘤坏死因子-α(TNF-α)、白细胞介素-2(IL-2)、白细胞介素-1β(IL-1β)水平以评估机体炎症反应.结果观察组的治疗总有效率为95.00%,高于对照组的77.50%,差异有统计学意义(P<0.05).治疗后,观察组的疼痛程度、日常生活活动、外展肌力、主动活动范围评分分别为(13.60±1.17)分、(17.82±2.14)分、(22.16±3.05)、(34.16±4.80)分,均高于对照组,组间差异有统计学意义(P<0.05).治疗后,观察组的TNF-α、IL-2、IL-1β水平均低于对照组,组间差异有统计学意义(P<0.05).结论肩周炎患者采用齐刺阿是穴配合温针灸治疗的效果确切,能改善肩关节功能与机体炎症反应,值得临床推广使用.

Objective To investigate the clinical effect of triple puncture Ashi point combined with warm acupuncture in the treatment of scapulohumeral periarthritis.Methods A total of 80 patients with scapulohumeral periarthritis admitted to our hospital from January 2019 to January 2022 were selected and were divided into two groups by random number table method.The control group(n=40)was treated with conventional acupuncture,and the observation group(n=40)was treated with triple puncture Ashi point combined with warm acupuncture on this basis.The clinical efficacy of the two groups was compared,the Constant-Murley scale was used to evaluate shoulder joint function,the levels of tumor necrosis factor-α(TNF-α),interleukin-2(IL-2)and interleukin-1β(IL-1β)before and after treatment were measured to evaluate the inflammatory response.Results The total effective rate of the observation group was 95.00%,which was higher than 77.50%of the control group,and the difference was statistically significant(P<0.05).After treatment,the scores of degree of pain,activities of daily living,abductor strength and active range of motion in the observation group were(13.60±1.17)points,(17.82±2.14)points,(22.16±3.05)points and(34.16±4.80)points,which were higher than those in the control group,and the differences between the groups were statistically significant(P<0.05).After treatment,the levels of TNF-α,IL-2 and IL-1βin the observation group were lower than those in the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion Triple puncture Ashi point combined with warm acupuncture in the treatment of scapulohumeral periarthritis has a definite effect,which can improve the function of shoulder joint and the inflammatory reaction of the body,and is worthy of clinical application.
