综合康复治疗对老年股骨转子间骨折术后患者功能恢复的影响探讨

Influence of Comprehensive Rehabilitation Treatment on Functional Recovery of Elderly Patients after Femoral Intertrochanteric Fractures Surgery

目的分析老年股骨转子间骨折术后患者采用综合康复治疗对其功能恢复的影响.方法将2020年6月—2021年6月该院收治的80例老年股骨转子间骨折术后患者纳为研究对象,按照随机数字表法分为观察组与对照组,各40例.对照组接受常规治疗,观察组接受综合康复治疗.比较两组患者术后髋关节功能恢复优良率、疼痛程度、术后骨折愈合时间、下床活动时间、患肢肿胀情况、生活质量以及术后并发症发生率.结果观察组患者术后髋关节功能恢复优良率为92.50%,高于对照组的65.00%,差异有统计学意义(P<0.05).两组患者术前、术后1 d的疼痛视觉模拟评分(VAS)评分比较,组间差异无统计学意义(P>0.05);术后6周,观察组VAS评分为(2.79±0.17)分,明显低于对照组的(4.08±0.21)分,差异有统计学意义(P<0.05).观察组患者术后骨折愈合时间、下床活动时间分别为(10.27±2.03)周、(40.38±3.07)d,均短于对照组的(11.83±2.03)周、(62.83±5.20)d,组间差异有统计学意义(P<0.05).术后12周,观察组患者大腿周径差为(2.04±0.27)cm,小于对照组的(3.12±0.33)cm,日常生活活动能力评分为(52.86±4.66)分,高于对照组的(45.99±3.98)分,组间差异有统计学意义(P<0.05).观察组术后并发症发生率为5.00%,明显低于对照组的25.00%,差异有统计学意义(P<0.05).结论综合康复治疗在促进老年股骨转子间骨折术后患者功能恢复方面的作用显著,能够有效减轻患者术后疼痛感,缩短其术后骨折愈合时间及下床活动时间,且有利于改善其日常生活能力、减少并发症发生,可在临床推广使用.

Objective To analyze the influence of comprehensive rehabilitation treatment on functional recovery in elderly patients after femoral intertrochanteric fractures surgery.Methods A total of 80 elderly patients after femoral intertrochanteric fractures surgery treated in this hospital from June 2020 to June 2021 were included as the research objects,and they were divided into an observation group and a control group according to the random number table method,with 40 cases in each group.The control group received conventional treatment,and the observation group received comprehensive rehabilitation treatment.The postoperative excellent and good rate of hip joint function recovery,pain degree,postoperative fracture healing time,time to get out of bed,swelling of affected limb,quality of life and incidence of postoperative complications were compared between the two groups.Results The excellent and good rate of postoperative hip joint function recovery in the observation group was 92.50%,which was higher than 65.00%in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the visual analogue scale(VAS)score of pain between the two groups before operation and 1 day after operation(P>0.05);6 weeks after operation,the VAS score of the observation group was(2.79±0.17)points,which was significantly lower than(4.08±0.21)points of the control group,and the difference was statistically significant(P<0.05).The postoperative fracture healing time and the time of getting out of bed in the observation group were(10.27±2.03)weeks and(40.38±3.07)days,respectively,which were shorter than(11.83±2.03)weeks and(62.83±5.20)days in the control group,and the differences between the groups were statistically significant(P<0.05).At 12 weeks after operation,the difference in thigh circumference of the observation group was(2.04±0.27)cm,which was smaller than(3.12±0.33)cm of the control group,the activity of daily living score was(52.86±4.66)points,which was higher than(45.99±3.98)points of the control group,and the differences between the groups were statistically significant(P<0.05).The incidence of postoperative complications in the observation group was 5.00%,which was significantly lower than 25.00%in the control group,and the difference was statistically significant(P<0.05).Conclusion Comprehensive rehabilitation treatment plays a significant role in promoting the functional recovery of elderly patients after femoral intertrochanteric fractures surgery,which can effectively reduce postoperative pain,shorten the postoperative fracture healing time and get out of bed,and it is beneficial to improve their daily life ability and reduce the occurrence of complications,and can be used in clinical promotion.
