高通量血液透析对老年慢性肾衰竭患者微炎症反应和肾功能的影响

Effect of High-Throughput Hemodialysis on Microinflammatory Response and Renal Function in Elderly Patients with Chronic Renal Failure

摘要 目的探讨老年慢性肾衰竭患者采用高通量血液透析对改善机体微炎症反应、肾功能的作用.方法选择本院2019年1月—2021年8月收治的老年慢性肾衰竭患者50例,以随机数字表法分组,对照组(n=25)采用常规血液透析治疗,观察组(n=25)采用高通量血液透析治疗,以治疗前后的白细胞介素-6(IL-6)、C反应蛋白(CRP)水平评估患者的微炎症反应状态;以治疗前后的血肌酐、尿素氮与血尿酸水平评估患者的肾功能;以治疗前后的血磷、血钙、钙磷乘积评估患者的钙磷代谢状态.结果治疗后,观察组的IL-6、CRP水平分别为(32.77±7.18)pg/mL、(8.04±2.17)mg/L,均低于对照组,组间差异有统计学意义(P<0.05).治疗后,观察组的血肌酐、尿素氮与血尿酸水平分别为(376.75±38.37)μmol/L、(7.28±2.05)mmol/L、(241.05±29.40)μmol/L,均低于对照组,组间差异有统计学意义(P<0.05).治疗后,观察组的血磷、钙磷乘积水平分别为(1.56±0.50)mmol/L、(42.56±6.03)mg^(2)/dL^(2),均低于对照组,组间差异有统计学意义(P<0.05).结论老年慢性肾衰竭者采用高通量血液透析治疗可减轻机体微炎症反应,提高肾功能,改善钙磷代谢. Objective To investigate the effect of high-throughput hemodialysis on improving microinflammatory response and renal function in senile patients with chronic renal failure.Methods A total of 50 elderly patients with chronic renal failure admitted to the hospital from January 2019 to August 2021 were divided into two groups by random number table method.The control group(n=25)was treated with routine hemodialysis,and the observation group(n=25)was treated with high-throughput hemodialysis.The levels of interleukin-6(IL-6)and C-reactive protein(CRP)before and after treatment were used to evaluate the status of microinflammatory response.The renal function was evaluated by serum creatinine,urea nitrogen and uric acid levels before and after treatment.The calcium and phosphorus metabolism status of patients was evaluated by the product of blood phosphorus,blood calcium and calcium phosphorus before and after treatment.Results After treatment,the IL-6 and CRP levels in the observation group were(32.77±7.18)pg/mL and(8.04±2.17)mg/L,respectively,lower than those in the control group,and the differences between the groups were statistically significant(P<0.05).After treatment,the serum creatinine,urea nitrogen and uric acid levels in the observation group were(376.75±38.37)μmol/L,(7.28±2.05)mmol/L and(241.05±29.40)μmol/L,respectively,lower than those in the control group,and the differences between the groups were statistically significant(P<0.05).After treatment,the product levels of blood phosphorus and calcium phosphorus in the observation group were(1.56±0.50)mmol/L and(42.56±6.03)mg^(2)/dL^(2),respectively,which were lower than those in the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion High-throughput hemodialysis can reduce microinflammatory response,improve renal function,and improve calcium and phosphorus metabolism in senile patients with chronic renal failure.
