呼吸功能康复训练结合噻托溴胺对慢性阻塞性肺疾病患者肺功能及运动耐力的影响

Effects of Respiratory Function Rehabilitation Training Combined with Tiotropium Bromide on Pulmonary Function and Exercise Tolerance in Patients with Chronic Obstructive Pulmonary Disease

摘要 目的探讨呼吸功能康复训练结合噻托溴胺对慢性阻塞性肺疾病(COPD)患者肺功能及运动耐力的影响.方法选取该院2020年2月—2021年2月收治的94例COPD患者,按随机数字表法分为对照组与观察组,各47例.对照组采用噻托溴胺干预,观察组在对照组基础上进行呼吸功能康复训练,均持续干预12周.对比两组的肺功能、呼吸肌生理学指标、运动耐力、生活质量.结果干预后,观察组的呼气流量峰值、第1秒用力呼气容积、用力肺活量、最大呼气压、最大吸气压均高于对照组,组间差异有统计学意义(P<0.05).干预后,观察组6 min步行试验距离为(413.84±17.36)m,长于对照组的(386.97±16.27)m,差异有统计学意义(P<0.05).干预后,观察组的生活质量综合评定问卷中心理、躯体、社会功能与物质状态评分均高于对照组,组间差异有统计学意义(P<0.05).结论呼吸功能康复训练结合噻托溴胺能够有效改善COPD患者的肺功能,促进患者呼吸功能恢复,从而显著的提高运动耐力及生活质量. Objective To investigate the effect of respiratory function rehabilitation training combined with tiotropium bromide on pulmonary function and exercise tolerance in patients with chronic obstructive pulmonary disease(COPD).Methods A total of 94 COPD patients admitted to our hospital from February 2020 to February 2021 were selected and divided into the control group and the observation group according to the random number table method,with 47 cases in each group.The control group was intervened with tiotropium bromide,and the observation group was given respiratory function rehabilitation training on the basis of the control group,and the intervention was continued for 12 weeks.The lung function,respiratory muscle physiology,exercise tolerance and quality of life were compared between the two groups.Results After intervention,the peak expiratory flow,forced expiratory volume in 1 second,forced vital capacity,maximal expiratory pressure and maximal inspiratory pressure in the observation group were higher than those in the control group,and the differences between the groups were statistically significant(P<0.05).After intervention,the 6-min walking test distance in the observation group was(413.84±17.36)m,which was longer than that in the control group(386.97±16.27)m,and the difference was statistically significant(P<0.05).After the intervention,the psychological,physical,social function and material status scores in the comprehensive assessment of quality of life in the observation group were higher than those in the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion Respiratory function rehabilitation training combined with tiotropium bromide can effectively improve the lung function of patients with COPD,promote the recovery of respiratory function of patients,and significantly improve exercise tolerance and quality of life.
