大剂量参附注射液治疗急性心力衰竭的临床研究

Clinical Study of High-Dose Shenfu Injection in the Treatment of Acute Heart Failure

目的探讨大剂量参附注射液在急性心力衰竭(AHF)患者中的治疗效果.方法选取该院2020年4月—2021年4月收治的50例AHF患者,按随机数字表法分为对照组与观察组,各25例.所有患者均采取常规抗心衰治疗,在此基础上,对照组加用常规剂量(50 mL)参附注射液,观察组加用大剂量(100 mL)参附注射液,均连续治疗1周.比较两组的临床疗效、生命体征、心功能、不良反应.结果观察组的治疗总有效率为92.00%,高于对照组的68.00%,差异有统计学意义(P<0.05).治疗前,两组的心率(HR)、呼吸频率(RR)相比,组间差异无统计学意义(P>0.05);治疗后,观察组的HR、RR分别为(72.37±5.32)次/min、(18.71±1.69)次/min,均低于对照组的(81.45±6.53)次/min、(24.36±2.24)次/min,组间差异有统计学意义(P<0.05).两组的左室收缩末期内径、左室舒张末期内径、左室射血分数、不良反应相比,组间差异无统计学意义(P>0.05).结论大剂量参附注射液在AHF患者的治疗中效果更佳,可改善患者生命体征,且无严重不良反应,安全可靠.

Objective To investigate the therapeutic effect of high-dose Shenfu injection in patients with acute heart failure(AHF).Methods A total of 50 patients with AHF who were admitted to our hospital from April 2020 to April 2021 were selected and divided into the control group and the observation group according to the random number table method,with 25 cases in each group.All patients were treated with conventional anti-heart failure treatment.On this basis,the control group was additionally given conventional dose(50 mL)Shenfu injection,and the observation group was additionally given high-dose(100 mL)Shenfu injection,all treated for 1 week.The clinical efficacy,vital signs,cardiac function and adverse reactions were compared between the two groups.Results The total effective rate of the observation group was 92.00%,which was higher than 68.00%of the control group,and the difference was statistically significant(P<0.05).Before treatment,there were no significant differences in heart rate(HR)and respiratory rate(RR)between the two groups(P>0.05);after treatment,the HR and RR of the observation group were(72.37±5.32)times/min and(18.71±1.69)times/min,respectively,which were lower than(81.45±6.53)times/min and(24.36±2.24)times/min of the control group,the differences between groups were statistically significant(P<0.05).There were no significant differences in left ventricular end-systolic diameter,left ventricular end-diastolic diameter,left ventricular ejection fraction and adverse reactions between the two groups(P>0.05).Conclusion High-dose Shenfu injection is more effective in the treatment of AHF patients,can improve the patient's vital signs,and has no serious adverse reactions,which is safe and reliable.
