补阳还五汤加减治疗后循环缺血性眩晕的临床疗效分析

Analysis of Clinical Efficacy of Modified Buyang Huanwu Decoction in the Treatment of Post-Circulatory Ischemic Vertigo

目的探讨补阳还五汤加减治疗后循环缺血性眩晕(PCIV)的临床效果.方法选取该院2020年5月—2021年5月收治的86例PCIV患者,按随机数表法划分成两组,各43例.对照组给予常规西医治疗,观察组在对照组基础上加用补阳还五汤加减治疗,连续治疗2周.对比两组临床疗效、中医症候积分、血液流变学指标、不良反应.结果观察组治疗总有效率93.02%高于对照组的76.74%,差异有统计学意义(P<0.05).治疗前,两组眩晕、头昏、呕吐积分与全血高切粘度、红细胞压积、全血低切粘度相比,组间差异无统计学意义(P>0.05);治疗后,观察组眩晕积分(1.02±0.25)分、头昏积分(1.16±0.41)分、呕吐积分(1.43±0.36)分及全血高切粘度(3.75±0.78)mpa·s、红细胞压积(40.38±6.76)%、全血低切粘度(6.53±1.29)mpa·s均低于对照组,组间差异有统计学意义(P<0.05).两组不良反应发生率相比,差异无统计学意义(P>0.05).结论补阳还五汤加减治疗PCIV效果确切,能够缓解临床症状,改善血液流变学,且无严重不良反应,值得临床推广应用.

Objective To investigate the clinical effect of modified Buyang Huanwu Decoction in the treatment of postcirculatory ischemic vertigo(PCIV).Methods 86 patients with PCIV admitted to the hospital from May 2020 to May 2021 were selected and divided into two groups according to the random number table method,with 43 cases in each group.The control group was given conventional western medicine treatment,and the observation group was additionally treated with modified Buyang Huanwu Decoction on the basis of the control group,with both treatments for 2 weeks.The clinical efficacy,TCM syndrome scores,hemorheological indexes and adverse reactions were compared between the two groups.Results The total effective rate 93.02%of the observation group was higher than 76.74%of the control group,with statistically significant difference(P<0.05).Before treatment,there was no statistically significant difference in the scores of vertigo,dizziness,vomiting,and the whole blood high shear viscosity,hematocrit,whole blood low shear viscosity between the two groups(P>0.05);after treatment,the vertigo score(1.02±0.25)points,dizziness score(1.16±0.41)points,vomiting score(1.43±0.36)points,whole blood high shear viscosity(3.75±0.78)mpa·s,hematocrit(40.38±6.76)%and whole blood low shear viscosity(6.53±1.29)mpa·s in the observation group were lower than those in the control group,and the differences between the groups were statistically significant(P<0.05).There was no statistically significant difference in incidence of adverse reactions between the two groups(P>0.05).Conclusion Modified Buyang Huanwu Decoction has definite effect in the treatment of patients with PCIV,can relieve clinical symptoms,improve blood rheology,and has no serious adverse reactions,which is worthy of clinical application.
