摘要
目的探讨电刺激生物反馈疗法结合阴道哑铃、凯格尔运动对产后盆底功能恢复的效果.方法选取本院2019年3月—2021年3月收治的70例产后盆底功能障碍患者为研究对象,根据随机数字表法分为两组.对照组(n=35)进行阴道哑铃训练及凯格尔运动,观察组(n=35)在对照组基础上联合盆底肌电刺激生物反馈疗法.比较两组干预前后的最大尿道闭合压、I类肌纤维肌力、Ⅱ类肌纤维肌力、盆底功能评分,观察两组的干预效果及预后情况.结果观察组的干预总有效率为91.43%,高于对照组的62.86%,差异有统计学意义(P<0.05).干预后,观察组的最大尿道闭合压、I类肌纤维肌力、Ⅱ类肌纤维肌力均高于对照组,组间差异有统计学意义(P<0.05).干预后,观察组的各项盆底功能评分均低于对照组,组间差异有统计学意义(P<0.05).干预后,观察组的子宫脱垂、性功能障碍、尿失禁发生率均低于对照组,组间差异有统计学意义(P<0.05).结论盆底肌电刺激生物反馈疗法结合阴道哑铃、凯格尔运动可提高产后盆底功能障碍患者的盆底肌功能,减轻临床症状,改善预后,效果显著,值得临床推广使用.
Objective To investigate the effect of electrostimulation biofeedback therapy combined with vaginal dumbbell and Kegel exercise on pelvic floor function recovery after delivery.Methods A total of 70 patients with postpartum pelvic floor dysfunction admitted to our hospital from March 2019 to March 2021 were selected as the research objects,and were divided into two groups according to random number table method.The control group(n=35)received vaginal dumbbell training and Kegel exercise,and the observation group(n=35)received pelvic floor electromyography biofeedback therapy on the basis of the control group.The maximum urethral closure pressure,class I muscle fiber strength,classⅡmuscle fiber strength and pelvic floor function scores were compared between the two groups before and after intervention,and the intervention effect and prognosis of the two groups were observed.Results The total effective rate of the observation group was 91.43%,which was higher than 62.86%of the control group,the difference was statistically significant(P<0.05).After intervention,the maximum urethral closure pressure,muscle strength of class I muscle fiber and classⅡmuscle fiber in the observation group were higher than those in the control group,and the differences between groups were statistically significant(P<0.05).After intervention,the pelvic floor function scores of the observation group were lower than those of the control group,and the differences between groups were statistically significant(P<0.05).After intervention,the incidences of uterine prolapse,sexual dysfunction and urinary incontinence in the observation group were lower than those in the control group,and the differences between groups were statistically significant(P<0.05).Conclusion Pelvic floor electromyographic biofeedback therapy combined with vaginal dumbbell and Kegel exercise can improve the pelvic floor muscle function of patients with postpartum pelvic floor dysfunction,reduce clinical symptoms,improve prognosis,and has significant effects,which is worthy of clinical application.
