腹腔镜胃肠穿孔修补术对胃肠穿孔患者术后胃肠功能恢复的影响

Effect of Laparoscopic Repair of Gastrointestinal Perforation on the Postoperative Recovery of Gastrointestinal Function in Patients with Gastrointestinal Perforation

目的探讨腹腔镜胃肠穿孔修补术治疗胃肠穿孔患者的效果.方法选取该院2020年4月—2021年4月诊治的94例胃肠穿孔患者,依据随机数字表法分为两组,各47例.对照组予以开腹胃肠穿孔修补术治疗,观察组予以腹腔镜胃肠穿孔修补术治疗,均观察至患者出院.对比两组手术相关指标、胃肠功能恢复情况、并发症.结果观察组术中出血量为(20.46±2.71)mL,少于对照组的(39.43±3.65)mL,手术、住院、术后排气、肠鸣音恢复、胃肠减压解除时间分别为(52.43±5.17)min、(7.19±1.45)d、(24.39±2.83)h、(13.56±1.89)h、(27.21±3.56)h,均短于对照组的(61.57±5.89)min、(11.26±2.03)d、(36.71±3.49)h、(21.57±2.52)h、(39.58±4.37)h,并发症发生率为4.26%(2/47),低于对照组的17.02%(8/47),组间差异有统计学意义(P<0.05).结论腹腔镜胃肠穿孔修补术治疗胃肠穿孔患者效果明显,可减少出血量,缩短手术、住院时间,促进患者术后胃肠功能恢复,且并发症少,值得临床大力推广.

Objective To investigate the effect of laparoscopic repair of gastrointestinal perforation in treatment of patients with gastrointestinal perforation.Methods 94 patients with gastrointestinal perforation diagnosed and treated in this hospital from April 2020 to April 2021 were selected and divided into two groups according to the random number table method,with 47 cases in each group.The control group was treated with open gastrointestinal perforation repair,and the observation group was treated with laparoscopic gastrointestinal perforation repair,and all patients were observed until they were discharged from the hospital.The operation related indexes,gastrointestinal function recovery and complications were compared between the two groups.Results The amount of intraoperative bleeding in the observation group was(20.46±2.71)mL,which was less than(39.43±3.65)mL in the control group,the time of operation,hospitalization,postoperative exhaust,bowel sound recovery,and gastrointestinal decompression release were(52.43±5.17)min,(7.19±1.45)d,(24.39±2.83)h,(13.56±1.89)h,(27.21±3.56)h,which were all shorter than(61.57±5.89)min,(11.26±2.03)d,(36.71±3.49)h,(21.57±2.52)h,(39.58±4.37)h in the control group,the incidence of complication was 4.26%(2/47),which was lower than 17.02%(8/47)in the control group,the differences between the groups were statistically significant(P<0.05).Conclusion Laparoscopic repair of gastrointestinal perforation has obvious effect in the treatment of patients with gastrointestinal perforation,which can reduce the amount of bleeding,shorten the operation and hospital stay,and promote the recovery of gastrointestinal function after operation.It has few complications,and is worthy of clinical promotion.
