续骨活血汤在胫骨平台前内侧塌陷骨折合并膝关节后外侧复合体损伤术后患者中的应用效果分析

Analysis of Application Effect of Xugu Huoxue Decoction in Patients with Anteromedial Collapse Fracture of Tibial Plateau Combined with Knee Joint Posterolateral Complex Injury after Operation

目的选择续骨活血汤在胫骨平台前内侧塌陷骨折合并膝关节后外侧复合体损伤术后患者中的应用效果.方法选取我院骨科2019年5月—2020年12月收治的82例胫骨平台前内侧塌陷骨折合并膝关节后外侧复合体损伤患者为研究对象,按照随机抽签法分为对照组与观察组,每组41例.两组均接受微创接骨术治疗,对照组术后辅以常规治疗联合功能锻炼,观察组在对照组基础上辅以续骨活血汤.对比两组术前及术后6个月膝关节功能、胫骨平台畸形状况以及术后并发症发生情况.结果术后6个月,观察组美国特种外科医院评分较对照组高,差异有统计学意义(P<0.05);观察组胫骨平台内翻角小于对照组,胫骨平台后倾角大于对照组,组间差异有统计学意义(P<0.05).观察组术后并发症发生率较对照组低,差异有统计学意义(P<0.05).结论续骨活血汤在胫骨平台前内侧塌陷骨折合并膝关节后外侧复合体损伤术后患者中的整体应用效果显著,可改善患者的膝关节功能,减少胫骨平台畸形及并发症发生,值得推广.

Objective To select the application effect of Xugu Huoxue decoction in postoperative patients with anteromedial collapse fracture of tibial plateau combined with knee joint posterolateral complex injury.Methods A total of 82 patients with anteromedial collapse fractures of the tibial plateau combined with knee joint posterolateral complex injury who were admitted to the department of orthopedics in our hospital from May 2019 to December 2020 were selected as the research subjects,and divided into a control group and an observation group by random drawing,with 41 cases in each group.Both groups received minimally invasive osteosynthesis,the control group was supplemented with routine treatment combined with functional exercise after surgery,and the observation group was supplemented with Xugu Huoxue decoction on the basis of the control group.The knee joint function,tibial plateau deformity before and 6 months after surgery and postoperative complications were compared between the two groups.Results 6 months after operation,the hospital for special surgery score of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05);the tibial plateau-tibial shaft angle in the observation group was smaller than that in the control group,and the posterior slope angle in the observation group was larger than that in the control group,and the differences between the groups were statistically significant(P<0.05).The incidence of postoperative complications in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion The overall application effect of Xugu Huoxue decoction in patients with anteromedial collapsed fracture of the tibial plateau combined with postoperative knee ioint posterolateral complex injury is remarkable,whicu can improve the knee joint function of patients,reduce the deformity of the tibial plateau and the occurrence of complications,and is worthy of promotion.
