带线锚钉联合克氏针固定重建在指伸肌腱中央束止点断裂患者中的应用效果观察

Observation of the Application Effect of Anchor Nails with Wire Combined with Kirschner Wire Fixation and Reconstruction in Patients with Rupture of the Central Bundle Insertion of the Extensor Digitorum Tendon

目的探讨带线锚钉联合克氏针固定重建在指伸肌腱中央束止点断裂患者中的应用效果.方法选取该院2019年5月—2020年11月收治的80例指伸肌腱中央束止点断裂患者为研究对象,按随机数字表法分为两组,各40例.对照组采用抽出钢丝法重建,观察组采用带线锚钉联合克氏针固定重建,均于术后随访6个月.对比两组手术相关指标、肢体功能恢复情况、并发症发生率.结果观察组手术时间、住院时间以及手部功能恢复时间分别为(31.57±4.26)min、(5.31±1.02)d、(15.49±1.83)周,均短于对照组的(48.69±5.37)min、(8.42±1.63)d、(18.63±2.17)周,肢体功能恢复优良率95.00%高于对照组的80.00%,组间差异有统计学意义(P<0.05).两组并发症发生率比较,差异无统计学意义(P>0.05).结论带线锚钉联合克氏针固定在指伸肌腱中央束止点断裂患者中的疗效确切,可有效缩短手术、住院时间,促进患者肢体功能恢复,且并发症较少,安全性较高.

Objective To investigate the application effect of anchor nails with wire combined with Kirschner wire fixation and reconstruction in patients with rupture of the central bundle of extensor digitorum tendon.Methods A total of 80 patients with rupture of the central bundle insertion of the extensor digitorum tendon in our hospital from May 2019 to November 2020 were selected as the research objects,and divided into two groups according to the random number table method,with 40 cases in each group.The control group was reconstructed by pulling out the wire method,and the observation group was reconstructed by anchor nails with wire combined with Kirschner wire fixation.Both groups were followed up for 6 months after operation.The operation-related indicators,limb function recovery,and complication rates were compared between the two groups.Results The operation time,hospital stay and hand function recovery time of the observation group were(31.57±4.26)min,(5.31±1.02)d,(15.49±1.83)weeks,respectively,which were shorter than(48.69±5.37)min,(8.42±1.63)d,(18.63±2.17)weeks of the control group,the excellent and good rate of limb function recovery was 95.00%,which was higher than 80.00%of the control group,and the differences between the groups were statistically significant(P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion Anchor nails with wire combined with Kirschner wire fixation in patients with rupture of the central bundle of extensor digitorum tendon has a definite curative effect,can effectively shorten the operation and hospitalization time,and promote the recovery of limb function of patients,with fewer complications and higher safety.
