桃红四物汤加减对四肢骨折患者骨代谢、微循环及疼痛肿胀程度的影响

Effect of Modified Taohong Siwu Decoction on Bone Metabolism, Microcirculation, Pain and Swelling in Patients with Limb Fracture

目的探讨桃红四物汤加减对四肢骨折患者骨代谢、微循环及疼痛肿胀程度的影响.方法选择2020年1—12月该院收治的79例四肢骨折患者为研究对象,根据随机数字表法分为观察组(40例)和对照组(39例).两组患者均行骨科内固定术,对照组患者采用骨伤科常规术后治疗,观察组患者在此基础上加用桃红四物汤加减治疗,两组均治疗21 d.对比两组患者治疗前后的患侧和健侧周径差,疼痛视觉模拟评分(visual analogue scale,VAS),血清Ⅰ型原胶原N端前肽(procollagenⅠtype N-terminal peptide,PINP)含量,骨钙素N端中分子(N-MID osteocalcin,N-MID)含量及血液微循环情况.结果治疗后,观察组患者的患侧和健侧周径差小于对照组,VAS评分明显低于对照组,组间差异有统计学意义(P<0.05).治疗后,观察组患者的血清PINP和N-MID含量均明显低于对照组,组间差异有统计学意义(P<0.05).治疗后,观察组患者的甲襞总积分明显低于对照组,毛细血管流速明显快于对照组,组间差异有统计学意义(P<0.05).结论桃红四物汤加减有助于减轻四肢骨折患者的肿胀和疼痛程度,促进血液微循环,改善机体骨代谢水平.

Objective To investigate the effect of modified Taohong Siwu Decoction on bone metabolism,microcirculation,pain and swelling in patients with limb fracture.Methods A total of 79 patients with limb fracture admitted to this hospital from January to December 2020 were selected as the research objects,and were divided into an observation group(40 cases)and a control group(39 cases)according to random number table method.Patients in both groups received orthopedic internal fixation.The control group received routine postoperative treatment in the department of orthopedics,and the observation group was additionally treated with modified Taohong Siwu Decoction.Both groups were treated for 21 days.The circumference difference of affected side and healthy side,visual analogue scale(VAS)score of pain,and serum procollagenⅠType N-terminal peptide(PINP),N-mid osteocalcin(N-mid)and blood microcirculation were compared between the two groups before and after treatment.Results After treatment,the peripheral diameter difference of the affected side and the healthy side in the observation group was smaller than the control group and the VAS score was significantly lower than that in the control group,the differences between the two groups were statistically significant(P<0.05).After treatment,serum PINP and N-mid contents in observation group were significantly lower than those in control group,the differences between the two groups were statistically significant(P<0.05).After treatment,the total nailfold score of the observation group was significantly lower than that of the control group,and the capillary flow rate was significantly faster than that of the control group,the differences between the two groups were statistically significant(P<0.05).Conclusion Modified Taohong Siwu Decoction can reduce swelling and pain,promote blood microcirculation and improve bone metabolism in patients with limb fracture.
