中药热奄包与穴位按摩结合情志护理对强直性脊柱炎患者自觉症状体征及心理状态的影响

Influence of Traditional Chinese Medicine Reambao and Acupoint Massage Combined with Emotional Nursing on Subjective Symptoms, Signs and Psychological State of Patients with Ankylosing Spondylitis

目的探讨中药热奄包与穴位按摩结合情志护理在强直性脊柱炎患者中的应用效果.方法选取该院2018年9月—2020年9月收治的106例强直性脊柱炎患者,按随机数字表法分为两组,每组53例.对照组采取常规护理,观察组在对照组基础上采取中药热奄包与穴位按摩结合情志护理.比较两组的自觉症状体征、心理状态、关节功能、炎性因子水平、生活质量和护理满意度.结果干预后,观察组的Bath强直性脊柱炎病情活动性指数评分、脊柱痛及夜间痛的视觉模拟评分、Bath强直性脊柱炎功能指数评分、抑郁自评量表评分、焦虑自评量表评分均低于对照组,健康调查简表评分高于对照组,组间差异有统计学意义(P<0.05).干预后,观察组的扩胸度大于对照组,颔胸距、指地距、枕墙距均短于对照组,组间差异有统计学意义(P<0.05).干预后,观察组的C反应蛋白、红细胞沉降率水平均低于对照组,组间差异有统计学意义(P<0.05).观察组的护理满意度为96.23%高于对照组的84.91%,组间差异有统计学意义(P<0.05).结论中药热奄包与穴位按摩结合情志护理在强直性脊柱炎患者中的应用效果良好,能够减轻患者腰背部疼痛等相关临床症状,缓解消极情绪,加快关节功能恢复及生活质量提升,从而获得更高的护理满意度.

Objective To explore the application effect of traditional Chinese medicine Ream Bao and acupoint massage combined with emotional nursing in patients with ankylosing spondylitis.Methods A total of 106 patients with ankylosing spondylitis admitted to our hospital from September 2018 to September 2020 were selected and divided into two groups according to the random number table method,with 53 cases in each group.The control group received routine nursing care,while the observation group received traditional Chinese medicine hot packs and acupoint massage combined with emotional nursing on the basis of the control group,The subjective symptoms and signs,psychological state,joint function,inflammatory factor levels,quality of life and nursing satisfaction were compared between the two groups.Results After the intervention,the Bath ankylosing spondylitis disease activity index scores,visual analogue scores for spinal pain and nocturnal pain,bath ankylosing spondylitis functional index scores,self-rating depression anxiety scale scores and self-rating anxiety scale scores were lower in the observation group than in the control group,and the MOS item short from health survey scores were higher than the control group,with the statistically significant differences between the groups(P<0.05).After the intervention,the chest expansion degree of the observation group was greater than that of the control group,and the chin-chest distance,finger-to-ground distance,and pillow-to-wall distance were all shorter than those of the control group,and the difference between the groups was statistically significant(P<0.05).After intervention,the levels of C-reactive protein and erythrocyte sedimentation rate in the observation group were lower than those in the control group,and the differences between the groups were statistically significant(P<0.05).The nursing satisfaction of the observation group was 96.23%,which was higher than 84.91%of the control group,and the difference was statistically significant(P<0.05).Conclusion The application of traditional Chinese medicine hot packs and acupoint massage combined with emotional nursing in patients with ankylosing spondylitis has a good effect,which canrelieve patients'low back pain and other related clinical symptoms,relieve negative emotions,speed up the recovery of joint function and improve the quality of life,so as to obtain higher nursing satisfaction.
