围术期系统性肺康复训练对肺癌行胸腔镜肺叶切除术后患者肺功能及呼吸功能的影响 被引量:1

摘要 目的探究围术期系统性肺康复训练对胸腔镜肺叶切除术后患者肺功能及呼吸功能的影响.方法选取该院2019年10月—2020年10月收治的108例肺癌行胸腔镜肺叶切除术后患者为研究对象,按照随机数字表法分为对照组和观察组,每组54例.对照组采用常规康复干预,观察组在对照组基础上采用围术期系统性肺康复训练.比较两组的肺功能、呼吸功能及运动功能.结果干预前,两组的各项肺功能指标比较,组间差异无统计学意义(P>0.05);干预后,观察组的第1秒用力呼气容积、第1秒用力呼气容积占用力肺活量比值均高于对照组,组间差异有统计学意义(P<0.05);两组的用力肺活量、最大通气量比较,组间差异无统计学意义(P>0.05).干预前,两组的呼吸及运动功能比较,组间差异无统计学意义(P>0.05);干预后,观察组的Borg评分低于对照组,6MWT长于对照组,组间差异有统计学意义(P<0.05).结论围术期系统性肺康复训练能够提高肺癌行胸腔镜肺叶切除术后患者的呼吸功能,增强运动耐力,促进肺功能改善,利于术后恢复. Objective To explore the effect of perioperative systemic pulmonary rehabilitation training on pulmonary function and respiratory function of patients with lung cancer after thoracoscopic lobectomy.Methods A total of 108 lung cancer patients after thoracoscopic lobectomy who were admitted to the hospital from October 2019 to October 2020 were selected as the research objects,and were divided into a control group and an observation group according to the random number table method,with 54 cases in each group.The control group used conventional rehabilitation intervention,and the observation group used perioperative systemic pulmonary rehabilitation training on the basis of the control group.The pulmonary function,respiratory function and motor function of the two groups were compared.Results Before the intervention,the lung function indicators of the two groups were compared,and the differences between the groups were not statistically significant(P>0.05);after the intervention,the forced expiratory volume in one second and the forced expiratory volume in one second occupies forced vital capacity in the observation group were higher than those in the control group,and the differences between the groups were statistically significant(P<0.05);there were no statistically significant differences between the two groups of forced vital capacity and maximal voluntary ventilation(P>0.05).Before the intervention,the respiratory and motor functions of the two groups were compared,and the differences between the groups were not statistically significant(P>0.05);after the intervention,the Borg score of the observation group was lower than that of the control group,and the 6 MWT was longer than that of the control group,the differences between the groups were statistically significant(P<0.05).Conclusion Perioperative systemic pulmonary rehabilitation training can improve the respiratory function of patients after thoracoscopic lobectomy for lung cancer,enhance exercise endurance,promote improvement of pulmonary function,and facilitate postoperative recovery.
