半髋与全髋关节置换术在老年股骨颈骨折治疗中的疗效对比

Comparison of Curative Effect Between Hemihip and Total Hip Arthroplasty in the Treatment of Femoral Neck Fracture in the Elderly

目的分析对比半髋与全髋关节置换术在老年股骨颈骨折治疗中的效果.方法选择该院于2019年5月—2020年7月收治的老年股骨颈骨折患者72例为研究对象,按照随机数字表法分成对照组和观察组,每组36例.对照组患者采取半髋关节置换术治疗,观察组患者采取全髋关节置换术治疗.观察并对比两组的治疗效果.结果观察组的治疗总优良率94.44%,较对照组77.78%的优良率高,差异有统计学意义(P<0.05).治疗前,两组的手术情况及髋关节功能评分(Harris hip score,HHS)对比,组间差异无统计学意义(P>0.05);治疗后,观察组患者的手术时间、髋关节恢复时间明显短于对照组,术中出血量明显少于对照组,HSS评分明显高于对照组,组间差异有统计学意义(P<0.05).观察组患者的并发症总发生率为5.56%,显著低于对照组患者的25.00%,差异有统计学意义(P<0.05).结论老年股骨颈骨折患者采用全髋关节置换术治疗的效果与半髋关节置换术相比,全髋关节置换术疗效更显著,手术时间和恢复时间更短,且髋关节功能恢复较好,并发症少,值得广泛使用.

Objective Analyze and compare the effects of hemi-hip and total hip arthroplasty in the treatment of elderly femoral neck fractures.Methods 72 elderly patients with femoral neck fractures admitted to the hospital from May 2019 to July 2020 were selected for the study,and they were divided into control and observation groups by the random number table method,with 36 cases in each group.The patients in the control group were treated with hemiarthroplasty and the patients in the observation group were treated with total hip arthroplasty.The treatment effects of the two groups were observed and compared.Results The overall excellent rate of treatment in the observation group was 94.44%,which was higher than the excellent rate of 77.78%in the control group,and the difference was statistically significant(P<0.05).Before treatment,the operation status and Harris Hip score of the two groups were compared,and the differences between the groups were not statistically significant(P>0.05);after treatment,the operation time and hip recovery time of patients in the observation group were significantly shorter than those in the control group,the intraoperative bleeding was significantly less than that in the control group,and the HSS score was significantly higher than that in the control group,and the differences between the groups were statistically significant(P<0.05).The total incidence of complications in the observation group was 5.56%,which was significantly lower than the 25.00%in the control group,and the difference was statistically significant(P<0.05).Conclusion The efficacy of total hip arthroplasty in elderly patients with femoral neck fractures compared with hemiarthroplasty is more significant,with the shorter operation time and recovery time,and the better recovery of hip function with fewer complications are worthy of widespread use.
