站立下行肩胛带运动控制训练对脑卒中后偏瘫患者上肢功能、平衡功能及日常生活活动能力的影响 被引量:1

Effects of Scapular Girdle Movement Control Training in Standing and Descending on Upper Limb Function, Balance Function and Activities of Daily Living in Patients with Hemiplegia after Stroke

摘要 目的研究站立下行肩胛带运动控制训练对脑卒中后偏瘫患者上肢功能、平衡功能及日常生活活动能力的影响.方法选取2020年3月—2021年2月该院收治的60例脑卒中后偏瘫患者为研究对象,采用随机数字表法分为观察组与对照组,每组30例.对照组患者采用传统康复训练,观察组在对照组基础上采用站立下行肩胛带运动控制训练.比较两组患者治疗前后上肢功能、平衡功能、日常生活能力.结果治疗后,两组的Fugl-Meyer运动功能评定量表上肢部分(Fugl-Meyer Motor Assessment-Upper Extremities,FMA-UE)、偏瘫上肢功能测试(香港版)(Hong Kong Edition of Functional Test for The Hemiplegic Upper Extremity,FTHUE-HK)、Fugl-Meyer平衡量表(balance subscale of the Fugl-Meyer Assessment,FMA-B)、Berg平衡量表(Berg balance scale,BBS)及改良Barthel指数评定量表(modified Barthel index,MBI)评分均较各组治疗前有所改善,差异有统计学意义(P<0.05).治疗后,观察组的FMA-UE、FTHUE-HK、FMA-B、BBS及MBI评分均优于对照组,组间差异有统计学意义(P<0.05).结论站立下行肩胛带运动控制训练可以显著改善脑卒中后偏瘫患者上肢功能、平衡功能及日常生活活动能力. Objective To study the effects of standing and descending scapular girdle movement control training on upper limb function,balance function and activities of daily living in patients with hemiplegia after stroke.Methods 60 patients with post-stroke hemiplegia admitted to the hospital from March 2020 to February 2021 were selected as the research objects.They were divided into an observation group and a control group by random number table method,with 30 cases in each group.The patients in the control group received traditional rehabilitation training,and the observation group used the control training of the scapular girdle movement while standing and descending on the basis of the control group.The upper limb function,balance function and ability of daily living were compared between the two groups of patients before and after treatment.Results After treatment,the Fugl-Meyer Motor Assessment-Upper Extremities(FMA-UE),Hong Kong Edition of Functional Test for The Hemiplegic Upper Extremity(FTHUE-HK),balance subscale of the Fugl-Meyer Assessment(FMAB),Berg balance scale(BBS)and modified Barthel index(MBI)scores of two groups were improved compared with those before treatment in each group,the differences between the groups were statistically significant(P<0.05).After treatment,the FMA-UE,FTHUE-HK,FMA-B,BBS and MBI scores of the observation group improved significantly compared with the control group,and the difference between the groups was statistically significant(P<0.05).Conclusion Standing and descending scapular girdle movement control training can significantly improve upper limb function,balance function and activities of daily living in patients with hemiplegia after stroke.
