肌骨超声定位下体外发散式冲击波疗法对膝关节术后软组织粘连患者关节功能康复的影响 被引量:1

Effect of Extracorporeal Divergent Shockwave Therapy with Musculoskeletal Ultrasound Localization on Joint Functional Rehabilitation of Patients with Postoperative Soft Tissue Adhesion of Knee Joint

摘要 目的探讨肌骨超声定位下体外发散式冲击波疗法在膝关节术后软组织粘连患者中的应用效果.方法选取2019年7月—2020年10月该院收治的膝关节术后软组织粘连患者82例为研究对象,按随机数字表法分为两组,各41例.对照组采用常规功能锻炼治疗,观察组在对照组基础上加用肌骨超声定位下体外发散式冲击波疗法治疗,均持续治疗4周.比较两组治疗前后膝关节活动优良率、膝关节功能、膝关节活动度、疼痛视觉模拟评分(visual analogue scale,VAS).结果观察组膝关节活动优良率高于对照组,差异有统计学意义(P<0.05).观察组治疗后日常生活、疼痛、关节僵硬评分及总分分别为(17.53±2.19)分、(2.61±0.45)分、(0.65±0.14)分、(21.26±3.26)分,均低于对照组的(23.44±3.62)分、(3.62±0.67)分、(1.02±0.29)分、(27.54±3.69)分,组间差异有统计学意义(P<0.05).观察组治疗后膝关节活动度为(113.96±10.64)°,大于对照组的(102.14±10.25)°,VAS评分为(2.13±0.35)分,低于对照组的(2.84±0.43)分,组间差异有统计学意义(P<0.05).结论膝关节术后软组织粘连患者采用肌骨超声定位下体外发散式冲击波疗法治疗,可增加关节活动度,减轻关节疼痛感,加快关节功能恢复. Objective To investigate the application effect of extracorporeal divergent shockwave therapy with musculoskeletal ultrasound localization in patients with soft tissue adhesions after knee joint surgery.Methods A total of 82 patients with soft tissue adhesions after knee joint surgery who were admitted to the hospital from July 2019 to October 2020 were selected as the research objects,and divided into two groups according to the random number table method,with 41 cases in each group.The control group was treated with routine functional exercise,and the observation group was treated with extracorporeal divergent shockwave therapy with musculoskeletal ultrasound localization on the basis of the control group,and the treatment continued for 4 weeks.The excellent and good rate of knee joint movement,knee joint function,knee joint range of motion and pain visual analogue scale(VAS)were compared between the two groups before and after treatment.Results The excellent and good rate of knee joint activity in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).The daily life,pain and joint stiffness scores and total scores of the observation group after treatment were(17.53±2.19)points,(2.61±0.45)points,(0.65±0.14)points,(21.26±3.26)points,respectively,which were lower than(23.44±3.62)points,(3.62±0.67)points,(1.02±0.29)points,(27.54±3.69)points of the control group,and the differences between the groups were statistically significant(P<0.05).After treatment,the range of motion of the knee joint in the observation group was(113.96±10.64)°,which was higher than(102.14±10.25)°of the control group,and the VAS score was(2.13±0.35)points,which was lower than(2.84±0.43)points of the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion In patients with soft tissue adhesions after knee joint surgery,extracorporeal divergent shockwave therapy with musculoskeletal ultrasound localization can increase the range of motion,reduce joint pain,and speed up the recovery of joint function.
