全程专业个案管理对青年糖尿病患者自我管理水平及康复效果的影响

Influence of Whole-Process Professional Case Management on Self-Management Level and Rehabilitation Effect of Young Patients with Diabetes

目的探讨全程专业个案管理在青年糖尿病患者中的应用效果.方法选择2019年5月—2020年5月于该院门诊就诊的青年糖尿病患者88例,按随机数字表法分为两组,各44例.对照组采用常规护理,观察组采用全程专业个案管理,两组均干预6个月.对比两组护理前后的自我管理水平、自护能力、血糖水平及护理满意度.结果观察组护理后自我管理水平中的知识、态度、行为评分分别为(90.93±3.98)分、(73.72±3.37)分、(137.71±12.89)分,均高于对照组,组间差异有统计学意义(P<0.05).观察组护理后自护能力评分为(135.36±8.44)分,高于对照组,差异有统计学意义(P<0.05).观察组护理后空腹血糖、餐后2 h血糖及糖化血红蛋白水平分别为(6.11±0.93)mmol/L、(6.79±0.74)mmol/L、(6.44±0.62)%,均低于对照组,护理满意度95.45%高于对照组的81.82%,组间差异有统计学意义(P<0.05).结论全程专业个案管理能够提高青年糖尿病患者的自我管理水平与自护能力,增强血糖控制效果,获得较好的康复效果,进而提高了患者的护理满意度.

Objective To explore the application effect of whole-process professional case management in young patients with diabetes.Methods A total of 88 young patients with diabetes who visited the outpatient clinic of this hospital from May 2019 to May 2020 were selected and divided into two groups according to the random number table method,with 44 cases in each group.The control group received routine nursing,while the observation group received whole-process professional case management.Both groups were given 6-month intervention.The self-management level,self-care ability,blood sugar level before and after nursing and nursing satisfaction were compared between the two groups.Results The scores of knowledge,attitude and behavior in the self-management level of the observation group after nursing were(90.93±3.98)points,(73.72±3.37)points,(137.71±12.89)points,which were higher than those of the control group,and the differences between the groups were statistically significant(P<0.05).The self-care ability score of the observation group after nursing was(135.36±8.44)points,which was higher than that of the control group,and the difference was statistically significant(P<0.05).The levels of fasting blood glucose,2 h postprandial blood glucose and glycosylated hemoglobin in the observation group after nursing were(6.11±0.93)mmol/L,(6.79±0.74)mmol/L,and(6.44±0.62)%,which were lower than those in the control group,the nursing satisfaction was 95.45%,which was higher than 81.82%of the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion Whole-process professional case management can improve the self-management level and self-care ability of young patients with diabetes,enhance the effect of blood sugar control,obtain better rehabilitation effect,and improve the nursing satisfaction of patients.
