早期康复治疗联合高压氧治疗对高血压脑出血微创术后患者神经功能和肢体运动功能的影响 被引量:2

Effects of Early Rehabilitation Therapy Combined with Hyperbaric Oxygen Therapy on Neurological Function and Limb Motor Function in Patients with Hypertensive Cerebral Hemorrhage after Minimally Invasive Surgery

摘要 目的 探究早期康复治疗联合高压氧治疗对高血压脑出血微创术后患者神经功能和肢体运动功能的影响.方法 选取2019年4月—2020年8月在该院接受微创术治疗的62例高血压脑出血患者为研究对象,按照随机数字表法分为两组,即试验组(n=31)和对照组(n=31).对照组患者在术后开展早期康复治疗,包括早期体位摆放与运动功能训练、电针治疗、语言功能训练等,试验组患者在对照组基础上联合高压氧治疗.对比两组的临床疗效.结果 两组治疗后脑水肿面积、美国国立卫生研究院卒中量表(National Institute of Health stroke scale,NIHSS)评分、Fugl-Meyer肢体运动功能评定量表(Fugl-Meyer assessment,FMA)评分均明显优于治疗前,且试验组脑水肿面积小于对照组,NIHSS评分低于对照组,FMA评分高于对照组,组间差异有统计学意义(P<0.05).治疗后,两组脑血管周围阻力、平均流速、平均流量均明显优于治疗前,且试验组脑血管周围阻力小于对照组,平均流速快于对照组,平均流量大于对照组,组间差异有统计学意义(P<0.05).结论 高血压脑出血微创术后患者采用早期康复治疗联合高压氧治疗,可增加脑血管血流,修复受损神经,改善肢体运动功能. Objective To explore the effect of early rehabilitation therapy combined with hyperbaric oxygen therapy on neurological function and limb motor function in patients with hypertensive cerebral hemorrhage after minimally invasive surgery.Methods A total of 62 patients with hypertensive cerebral hemorrhage who received minimally invasive surgery in the hospital from April 2019 to August 2020 were selected as the research objects,and divided into two groups according to the random number table method,namely an experimental group(n=31)and a control group(n=31).The patients in the con-trol group were given early rehabilitation therapy after surgery,ineluding early postural placement and motor funetion training,electro-acupuncture therapy,language function training,etc.The patients in the experimental group were given hyperbaric oxygen therapy on the basis of the control group.The clinical efficacy of the two groups was compared.Results The brain edema area,National Institute of Health stroke scale(NIHSS)score and Fugl-Meyer assessment(FMA)score after treatment in the two groups were significantly better than those before treatment,and the brain edema area of the experimental group was smaller than that of the control group,the NIHSS score was lower than that of the control group,and the FMA seore was higher than that of the control group,and the differences between the groups were statistically significant(P<0.05).After treat-ment,the cerebral perivascular resistance,average flow velocity and average flow in the two groups were significantly better than those before treatment,and the cerebral perivascular resistance in the experimental group was lower than that in the con-trol group,the average flow rate was faster than that in the control group,and the average flow rate was greater than that in the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion Early rehabilita-tion therapy combined with hyperbaric oxygen therapy in patients with hypertensive cerebral hemorrhage after minimally inva-sive surgery can increase cerebrovascular blood flow,repair damaged nerves,and improve limb motor function.
