改良强制性运动疗法在脑卒中后偏瘫康复治疗中的应用效果分析 被引量:3

Analysis of the Application Effect of Modified Constraint-Induced Movement Therapy in the Rehabilitation Treatment of Hemiplegia after Stroke

摘要 目的分析改良强制性运动疗法(modified constraint-induced movement therapy,mCIMT)在脑卒中后偏瘫康复治疗中的应用效果.方法选取该院2020年1—11月收治的脑卒中后偏瘫患者132例,按照信封法分为对照组(n=66)与优化组(n=66).对照组采用综合康复治疗,优化组在对照组基础上采用mCIMT治疗.对比两组治疗前后的Fugl-Meyer运动功能评定(Fugl-Meyer assessment,FMA)、改良Barthel指数(modified Barthel index,MBI)、Berg平衡量表(Berg balance scale,BBS)、美国国立卫生研究院卒中量表(National Institute of Health stroke scale,NIHSS)评分及6 min步行距离.结果治疗后,优化组的FMA、MBI、BBS评分高于对照组,NIHSS评分低于对照组,组间差异有统计学意义(P<0.05).治疗4、8周后,两组的6 min步行距离均较治疗前有所改善,且优化组的6 min步行距离长于对照组,组间差异有统计学意义(P<0.05).结论在脑卒中后偏瘫患者康复治疗中应用mCIMT,可显著提高患者日常生活活动能力、运动功能及平衡能力,有效改善神经功能缺损症状,适宜推广. Objective To analyze the application effect of modified constraint-induced movement therapy(mCIMT)in the rehabilitation treatment of hemiplegia after stroke.Methods A total of 132 patients with post-stroke hemiplegia admitted to the hospital from January to November 2020 were selected and divided into a control group(n=66)and an optimized group(n=66)according to the envelope method.The control group was treated with comprehensive rehabilitation treatment,and the optimized group was treated with mCIMT on the basis of the control group.The Fugl-Meyer assessment(FMA),modified Barthel index(MBI),Berg balance scale(BBS),National Institute of Health stroke scale(NIHSS)score and 6 min walking distance before and after treatment between the two groups were compared.Results After treatment,the FMA,MBI,and BBS scores of the optimized group were higher than those of the control group,and the NIHSS score was lower than that of the control group,the differences between the groups were statistically significant(P>0.05).After 4 and 8 weeks of treatment,the 6 min walking distance of the two groups was improved compared with that before treatment,and the 6 min walking distance of the optimized group was longer than that of the control group,the differences between the groups were statistically significant(P<0.05).Conclusion The application of mCIMT in the rehabilitation treatment of patients with hemiplegia after stroke can significantly enhance the patients'activities of daily living,motor function and balance ability,can effectively improve the symptoms of neurological deficits,and is suitable for promotion.
