高频重复经颅磁刺激联合认知康复训练治疗脑卒中后认知障碍的效果评价

The Effect Evaluation of High-Frequency Repetitive Transcranial Magnetic Stimulation Combined with Cognitive Rehabilitation Training in the Treatment of Cognitive Impairment after Stroke

摘要 目的分析高频重复经颅磁刺激联合认知康复训练治疗脑卒中后认知障碍的效果.方法选取该院2018年11月—2019年11月收治的脑卒中后认知障碍患者80例,根据随机数字表法分为两组,每组40例.对照组采用认知康复训练,实验组在对照组基础上采用高频重复经颅磁刺激,共治疗4周.于4周后,比较两组患者的认知功能、生活能力及行为记忆力.结果实验组的MMSE评分(25.28±2.36)分、MoCA评分(23.56±3.71)分均高于对照组的(20.41±2.47)分、(19.54±3.66)分,组间差异有统计学意义(P<0.05).实验组的Barthel指数评分(50.26±5.71)分高于对照组的(41.35±5.36)分,差异有统计学意义(P<0.05).实验组的RBMT评分(16.31±1.24)分高于对照组的(10.71±1.47)分,差异有统计学意义(P<0.05).结论脑卒中认知障碍患者采用高频重复经颅磁刺激联合认知康复训练,可以取得理想的治疗效果,值得临床推广应用. Objective To analyze the effect of high-frequency repetitive transcranial magnetic stimulation combined with cognitive rehabilitation training in the treatment of cognitive impairment after stroke.Methods A total of 80 patients with post-stroke cognitive impairment admitted to the hospital from November 2018 to November 2019 were selected and divided into two groups according to the random number table method,with 40 cases in each group.The control group used cognitive rehabilitation training,and the experimental group used high-frequency repetitive transcranial magnetic stimulation on the basis of the control group.Both groups were treated for 4 weeks.After 4 weeks of treatment,the cognitive function,life ability and behavioral memory of the two groups of patients were compared.Results The MMSE score of the experimental group was(25.28±2.36)points and the MoCA score was(23.56±3.71)points,which were higher than the control group's(20.41±2.47)points and(19.54±3.66)points,and the differences between the two groups were statistically significant(P<0.05).The Barthel index score of the experimental group(50.26±5.71)points was higher than(41.35±5.36)points of the control group,and the difference was statistically significant(P<0.05).The RBMT score of the experimental group(16.31±1.24)points was higher than(10.71±1.47)points of the control group,the difference was statistically significant(P<0.05).Conclusion Post-stroke cognitive impoarment patients using high-frequency repetitive transcranial magnetic stimulation combined with cognitive rehabilitation training can achieve ideal therapeutic effects and is worthy of clinical application.
