集束化康复护理在脑梗死患者吞咽困难中的护理效果研究

Study on the Effect of Cluster-based Rehabilitation Nursing in Patients with Cerebral Infarction with Dysphagia

目的探究集束化康复护理在脑梗死患者吞咽困难中的护理效果.方法选择2017年5月—2019年5月胜利油田中心医院收治的脑梗死患者60例,随机分成观察组(n=30)和对照组(n=30).对照组采用常规护理,观察组在常规护理上给予患者集束化康复护理.护理12周后,比较两组的护理效果、QOL评分、神经缺损评分及日常生活能力.结果护理后,观察组吞咽困难、轻度困难的比例低于对照组,不困难的比例高于对照组,差异有统计学意义(P<0.05).护理后,两组的QOL评分及Barthel指数均高于护理前,且观察组指标均高于对照组,差异有统计学意义(P<0.05).护理后,两组的NIHSS评分均低于护理前,且观察组评分低于对照组,差异有统计学意义(P<0.05).结论集束化康复护理应用与脑梗死患者吞咽困难中,可缓解吞咽困难,持续提高患者生活质量,值得推广应用.

Objective To explore the nursing effect of cluster-based rehabilitation nursing intervention in patients with cerebral infarction with dysphagia.Methods 60 patients with cerebral infarction admitted to Shengli Oilfield Central Hospital from May 2017 to May 2019 were selected and randomly divided into an observation group(n=30)and a control group(n=30).The control group adopted routine nursing,and on this basis,the observation group adopted cluster-based rehabilitation nursing.After 12 weeks of nursing,the effects of the patients were evaluated,and the nursing effects,QOL scores,neurological deficit scores and daily living abilities of the two groups were compared.Results After nursing,the proportion of dysphagia and mild difficulty in the observation group was lower than that of the control group,and the proportion of not having difficulty was higher than that of the control group.The differences were statistically significant(P<0.05).After nursing,the QOL scores and Barthel index of the two groups were higher than before nursing,and the indexes of the observation group were higher than those of the control group,the differences were statistically significant(P<0.05).After nursing,the NIHSS scores of the two groups were lower than before nursing,and the scores of the observation group were lower than those of the control group,the difference was statistically significant(P<0.05).Conclusion The application of cluster-based rehabilitation nursing in patients with cerebral infarction dysphagia can relieve dysphagia and continuously improve the quality of life of the patients,which is worthy of popularization and application.
