高频彩超对高血压患者颈动脉斑块检查的应用及预后评估

The Application of High-frequency Color Doppler Ultrasound in the Detection of Carotid Plaque in Patients with Hypertension and the Evaluation of Prognosis

目的研究高频彩超对高血压患者颈动脉斑块检查的应用及预后评估.方法纳入2018年4月—2019年11月期间在定襄县人民医院接受治疗的120例高血压患者进行研究,根据患者颈动脉硬化的不同程度分为重度组50例和轻中度组70例,评估两组患者的收缩压、舒张压、高血压病程以及重度组患者的动脉硬化斑块分型.结果结果表明,轻中度组患者收缩压、舒张压和脉压差相较于重度组更低,重度组患者病程普遍高于15年,轻中度患者病程以6~15年较为常见,差异有统计学意义(P<0.05);重度患者颈动脉硬化斑块分型中,低于50岁、50~60岁的患者硬斑所占比例最高,超过60岁后以软斑块最为常见.结论患者随着年龄的增加,其不稳定颈动脉斑块也会增加,其中高血压病程、舒张压和颈动脉硬化程度有相关性,对高血压患者进行高频彩超检查能够为其预后康复评估作参考,值得应用.

Objective To study the application of high-frequency color Doppler ultrasound in the detection of carotid plaque in patients with hypertension and the evaluation of prognosis.Methods 120 patients with hypertension who were treated in Dingxiang County People's Hospital from April 2018 to November 2019 were included in the study.According to the different degrees of patients with stiff arteriosclerosis,they were divided into 50 severe group and 70 mild moderate group.The systolic blood pressure,diastolic blood pressure,the course of hypertension and the atherosclerotic plaque type of severe group were evaluated.Results The results showed that the systolic blood pressure,diastolic blood pressure and pulse pressure in the mild to moderate group were lower than those in the severe group,the course of hypertension in the severe group was generally more than 15 years,the course of hypertension in the mild to moderate group was more common from 6 to 15 years,the differences were statistically significant(P<0.05).The proportion of hard plaques in the severe group was the highest in the patients under 50 and 50-60 years old,and soft plaques were the most common after 60 years old.Conclusion With the increase of age,the unstable carotid plaque will also increase.The course of hypertension,diastolic blood pressure and the degree of carotid atherosclerosis are related.High-frequency color Doppler ultrasound can be used as a reference for the prognosis and rehabilitation of patients with hypertension.
