益肝灵片联合还原型谷胱甘肽治疗老年患者药物性肝损伤的疗效观察

Effect of Yiganling tablets combined with reduced glutathione on drug-induced liver injury in elderly patients

目的:观察益肝灵片联合还原型谷胱甘肽治疗老年患者药物性肝损伤的临床疗效.方法:选取80例老年患者随机分为观察组和对照组各40例,对照组给予还原型谷胱甘肽治疗,观察组在对照组的基础上再给予益肝灵片,疗程均为20天.疗程结束后比较两组的总有效率.同时比较治疗前后两组患者炎症因子血清白介素-2(IL-2)、白介素-10(IL-10)和肿瘤坏死因子-α(TNF-α)、肝功能指标血清谷丙转氨酶(ALT)、谷草转氨酶(AST)、总胆红素(TBIL)和谷氨酰转肽酶(GGT)和肝纤维化指标层粘连蛋白(LN),III型前胶原(PC-III),IV型胶原(IV-C)及透明质酸酶(HA)水平的变化.结果:观察组和对照组的总有效率分别为92.5%和67.6%,观察组总有效率明显高于对照组,差异有统计学意义;治疗组治疗后血清IL-2和TNF-α两者的水平比对照组显著性地降低,IL-10水平比对照组显著性地升高;治疗后两组肝功能指标血清ALT、AST、TBIL和GGT水平和LN、PC-III、IV-C及HA水平均明显下降,且治疗组下降程度明显高于对照组.结论:益肝灵片联合还原型谷胱甘肽治疗老年患者药物性肝损伤的临床疗效显著优于单独使用还原型谷胱甘肽.

Objective To observe the clinical efficacy of Yiganling tablets combined with reduced glutathione in the treatment of drug-induced liver injury in elderly patients.Methods 80 elderly patients were randomly divided into observation group and control group,40 cases in each group.The control group was treated with reduced glutathione,while the observation group was treated with Yiganling tablet on the basis of the control group,and the course of treatment was 20 days.The total effective rate of the two groups was compared after the end of treatment.At the same time,the levels of inflammatory factors:interleukin-2(IL-2),interleukin-10(IL-10),tumor necrosis factor-α(TNF-α),liver function indicators:levels of alanine aminotransferase(ALT),glutathione transaminase(AST),total bilirubin(TBIL),glutamyl transpeptidase(GGT)and liver fibrosis indicators:laminin(LN),procollagen III(PC-III),type IV collagen(IV-C)and hyaluronidase(HA)in serum were compared between the two groups before and after treatment.Results The total effective rates of the observation group and the control group were 92.5%and 67.6%,respectively.The total effective rates of the observation group were significantly higher than those of the control group.After treatment,the levels of serum IL-2 and TNF-αin the treatment group were significantly lower than those in the control group,and the levels of IL-10 were significantly higher than those in the control group.After treatment,the levels of ALT,AST,TBIL and GGT in serum and LN,PC-III,IV-C and HA in the two groups decreased significantly,and the degree of decline in the treatment group was significantly higher than that in the control group.Conclusion Yiganling tablets combined with reduced glutathione in the treatment of elderly patients with drug-induced liver injury is significantly better than that of reduced glutathione alone.
