摘要
目的:探究三维斑点追踪在子痫前期患者心脏功能检查中的应用价值.方法:以2017年2月~2018年10月在我院进行三维超声斑点追踪成像技术检查的80例单胎子痫前期患者作为病例组,并收集同期检查的正常单胎妊娠者80例作为对照组.比较两组患者的常规心脏超声参数以及整体纵向、整体圆周、整体面积及整体径向的应变指标.结果:病例组的左心房前后径(LAD)为(37.61±12.18)mm,大于对照组的(30.52±8.53)mm,其余指标包括室间隔舒张期厚度(IVSD)、左心室后壁舒张期厚度(LVPWD)、左心室舒张期内径(LVEDD)、左心室射血分数(LVEF)则无统计学差异.两组患者整体纵向及径向应变比较,病例组的侧壁(Lat)、后间隔(Sep)、前壁(Ant)、下壁(Inf)和后壁(Pos)分别为(10.98±5.25)%及(41.93±11.20)%、(11.41±5.18)%及(33.51±9.28)%、(13.47±6.36)%及(41.17±10.32)%、(11.55±7.63)%及(31.15±8.51)%、(10.61±5.81)%及(31.20±7.83)%低于对照组的(12.96±6.14)%及(55.27±14.21)%、(14.35±6.21)%及(38.06±10.19)%、(19.54±8.95)%及(57.92±12.58)%、(14.82±9.14)%及(40.28±9.74)%、(15.37±8.34)%及(43.18±8.16)%,差异具有统计学意义,两组间的前间隔(A-s)则无统计学差异.整体圆周应变指标比较,两组间的Sep、Lat、Ant、Inf、Pos、A-s均无统计学差异.整体面积应变比较,病例组的Ant、Inf、Pos分别为(26.94±8.17)%、(21.14±8.04)%、(18.79±7.41)%,低于对照组的(35.02±9.28)%、(26.81±8.19)%、(27.91±8.02)%,差异具有统计学意义,Sep、Lat、A-s则无统计学差异.结论:子痫前期妊娠者心脏负荷增加,心肌应变指标下降,三维斑点追踪对其心脏损伤的识别具有较高的诊断价值.
Objective To explore the application value of three-dimensional speckle tracing in cardiac function examination of preeclampsia patients.Methods 80 single fetal preeclampsia patients were selected as the case group and 80 normal single pregnancy cases were selected as the control group,which were examined by three-dimensional speckle tracing in our hospital from February 2017 to October 2018.The conventional echocardiographic parameters and the strain indexes of the whole longitudinal,the whole circumference,the whole area and the whole radial were compared between the two groups.Results The left atrial anteroposterior diameter(LAD)in the case group was(37.61±12.18)mm,which was higher than that in the control group[(30.52±8.53)mm],and the other indexes included interventricular septal diastolic thickness(IVSD),left ventricular posterior wall diastolic thickness(LVPWD),left ventricular diastolic diameter(LVEDD),Left ventricular ejection fraction(LVEF)had no significant differences.Comparison of the strain indexes of the whole longitudinal and the whole radial,the Sep,Lat,Ant,Inf and Pos in the case group were(10.98±5.25)%and(41.93±11.20)%,(11.41±5.18)%and(33.51±9.28)%,(13.47±6.36)%and(41.17±10.32)%,(11.55±7.63)%and(31.15±8.51)%.(10.61±5.81)%and(31.20±7.83)%,significantly lower than those in the control group[(12.96±6.14)%and(55.27±14.21)%,(14.35±6.21)%and(38.06±10.19)%,(19.54±8.95)%and(57.92±12.58)%,(14.82±9.14)%and(40.28±9.74)%,(15.37±8.34)%and(43.18±8.16)%],but there was no significant difference in A-s between the two groups.Compared of the whole circumference,there were no significant differences in Sep,Lat,Ant,Inf,Pos,A-s between the two groups.Compared of the whole area strain,the Ant,Inf,Pos in the case group were(26.94±8.17)%,(21.14±8.04)%,(18.79±7.41)%,which were lower than those in the control group[(35.02±9.28)%,(26.81±8.19)%,(27.91±8.02)%],but the Sep,Lat,A-s had no statistical differences.Conclusion In preeclampsia,cardiac load increases and myocardial strain index decreases.Three-dimensional speckle tracing is effective and noninvasive in evaluating cardiac injury.Three-dimensional speckle tracing;preeclampsia;cardiac function;echocardiography.
