血清富半胱氨酸蛋白61在高龄急性心力衰竭患者中的预后价值研究 被引量:2

The prognostic value of serum cysteine-rich protein 61 in elderly acute heart failure patients

摘要 目的 研究血清富半胱氨酸蛋白61(cysteine-rich protein 61,CYR61)在高龄急性心力衰竭(AHF)患者中的预后价值.方法 连续入选2014年6月至2017年9月在中山大学第一附属医院住院的年龄≥70岁的AHF患者128例,测定其入院时血清CYR61水平,用Pearson相关性分析方法探究CYR61水平与年龄的相关性.随后对患者进行3个月随访,用logistic回归分析方法探讨CYR61水平对3个月全因死亡的预测价值.结果 118例患者完成随访,其中37例死亡,全因死亡率为31.4%.相关性分析显示,CYR61水平与年龄无关.多因素回归分析显示,CYR61是3个月全因死亡的独立预测因子(比值比64.58,95%可信区间3.99~1045.11,P<0.01).结论 血清CYR61是预测高龄AHF患者短期预后的良好指标之一. Objective To investigate the prognostic value of serum cysteine-rich protein 61(CYR61)in elderly acute heart failure(AHF)patients.Methods 128 consecutive patients hospitalized with AHF aged over 70 years in the First Affiliated Hospital of Sun Yat-sen University from June 2014 to September 2017 were included.The serum CYR61 levels at admission were measured and the correlation between CYR61 levels and ages was analyzed by Pearson correlation analysis.The patients were followed up for 3 months and the primary outcome was defined as all-cause mortality.The predictive value of CYR61 level for the outcome was analyzed by logistic regression analysis.Results 118 patients finished follow-up,of which 37 died,so the all-cause mortality was 31.4%.Pearson correlation analysis showed that CYR61 level was not associated with age.CYR61 was an independent predictor of 3-month all-cause mortality in multivariate logistic regression analysis(odds ratio(OR)64.58,95%confidence interval(CI)3.99-1045.11,P<0.01)Conclusion Serum CYR61 was a good predictor for predicting the short-term prognosis in elderly patients with AHF.
