血液灌流联合连续性血液滤过对高脂血症性急性胰腺炎患者血脂水平、血清淀粉酶、脂肪酶及CRP水平的影响

Effects of hemoperfusion combined with continuous hemofiltration on levels of blood lipid, serum amylase, lipase and CRP in patients with hyperlipidemic acute pancreatitis

目的:研究高脂血症性急性胰腺炎患者使用血液灌流联合连续性血液滤对血脂水平、血清淀粉酶、脂肪酶及CRP水平影响.方法:选择我院诊治高脂血症性急性胰腺炎患者80例,随机分成单一组(n=40)和联合组(n=40),两组患者均实施常规治疗,其中单一组使用连续性血液滤过,联合组使用血液灌流联合连续性血液滤过.观察两组治疗前后血脂水平(TG)、血清淀粉酶、脂肪酶及C反应蛋白水平(CRP水平)、急性生理与慢性健康状况评分Ⅱ(APACHEⅡ)量表评分及Balthazar CT分级评分系统评分,治疗后腹胀缓解时间、腹痛缓解时间、血液滤过滤器使用时间及住院时间,记录相关不良反应.结果:治疗后,联合组TG、血清淀粉酶、脂肪酶、CRP水平较单一组均明显改善,差异有统计学意义;治疗后,联合组血液滤过滤器使用时间明显高于单一组,腹胀缓解时间、腹痛缓解时间、住院时间均明显短于单一组,差异有统计学意义.治疗后,联合组APACHEⅡ、Balthazar CT评分较单一组均降低,差异有统计学意义.结论:高脂血症性急性胰腺炎患者使用血液灌流联合连续性血液滤过治疗能明显改善患者血脂水平、血清淀粉酶、脂肪酶及CRP水平,缩短腹胀、腹痛及住院时间.

Objective To study the effects of hemoperfusion combined with continuous hemofiltration on levels of blood lipid,serum amylase,lipase and CRP in patients with hyperlipidemic acute pancreatitis.Methods 80 cases of patients with hyperlipidemic acute pancreatitis who were treated in our hospital were selected and randomly divided into single group(n=40)and combined group(n=40).The two groups were given routine treatment,and single group was given continuous hemofiltration,and combined group was given hemoperfusion combined with continuous hemofiltration.Blood lipid level(TG),serum amylase,lipase and C-reactive protein(CRP)levels,acute physiology and chronic health evaluation II(APACHE II)scale score and Balthazar CT grading score system score before and after treatment,and relief time of abdominal distension,relief time of abdominal pain,use time of blood filter and hospital stay after treatment were observed in the two groups.And the related adverse reactions were recorded.Results After treatment,the levels of TG,serum amylase,lipase and CRP in combined group were significantly improved compared with those in single group.After treatment,the use time of blood filter in combined group was significantly higher than that in single group,and the relief time of abdominal distension,relief time of abdominal pain and hospital stay were significantly shorter than those in single group.After treatment,the scores of APACHE II and Balthazar CT in combined group were lower than those in single group.Conclusion Hemoperfusion combined with continuous hemofiltration for patients with hyperlipidemic acute pancreatitis can significantly improve blood lipid level,serum amylase,lipase and CRP levels,and shorten abdominal distension,abdominal pain and hospital stay.
