彩色多普勒超声对胆囊腺瘤与胆囊癌的鉴别诊断价值

Value of color Doppler ultrasound in differential diagnosis of gallbladder adenoma and gallbladder carcinoma

目的 分析彩色多普勒超声在胆囊腺瘤与胆囊癌鉴别诊断中的应用价值.方法 选取2017年1月至2018年12月于千佛山医院检查并确诊的胆囊腺瘤和胆囊癌患者105例,根据病理结果,将其分为胆囊腺瘤组50例和胆囊癌组55例.所有患者均进行彩色多普勒超声检查,分析超声图像及检查结果.结果 胆囊癌组患者无壁内结石,血流丰富,浆膜层不完整比例较高;胆囊腺瘤组患者结石可位于腔内,也可出现于壁内,血流不丰富,浆膜层完整,两组比较差异有统计学意义(P<0.05);胆囊癌组患者的病灶最大直径、最大血流速度均大于胆囊腺瘤组患者,且血流等级Ⅲ级比例更高(P<0.05).结论 彩色多普勒超声能够为胆囊腺瘤、胆囊癌的鉴别诊断提供重要的影像学信息,具有较高的应用价值.

Objective To evaluate the value of color Doppler ultrasound in the differential diagnosis of gallbladder adenoma and gallbladder carcinoma.Methods A total of 105 cases of gallbladder adenoma and gallbladder carcinoma patients diagnosed in Qianfoshan Hospital from January 2017 to December 2018 were selected.And they were divided into gallbladder adenoma group(55 cases)and gallbladder carcinoma group(50 cases),according to the pathological results.All patients were examined by color Doppler ultrasound for comparison and analysis on ultrasound image and examination results.Results The ultrasound image of patients in gallbladder carcinoma group showed no intramural stones,but abundant blood flow and high proportion of incomplete serous layer.And the ultrasound image of gallbladder adenoma group showed blood flow was not abundant,the serosa was intact;the difference between the two groups was significant(P<0.05).The maximal diameter and velocity of the lesions in gallbladder carcinoma group were significantly larger than those in gallbladderadenoma group,and the proportion of gradeⅢblood flow was higher than that in gallbladder adenoma group(P<0.05).Conclusions Color Doppler ultrasound can provide important imaging information for differential diagnosis of gallbladder adenoma and gallbladder carcinoma,so the ultrasound examination has clinical value in diagnosis of theses diseases.
