摘要
目的 应用多体素1H-MRS,探讨阿尔茨海默病(AD)患者扣带回左右非对称性代谢的特点.方法 对20例AD患者、16例轻度认知功能损害(MCI)患者及17名正常健康老年人(NC)行多体素1H-MRS检查,检测并计算左、右扣带回N-乙酰天门冬氨酸(NAA)/肌醇(mI)、NAA/肌酸复合物(Cr)、胆碱复合物(Cho)/Cr?和mI/Cr,利用SPSS?17.0统计软件对其检测数据进行统计分析.结果 NC组左前扣带回与右前扣带回、左后扣带回与右后扣带回1H-MRS代谢物水平均无非对称性改变;MCI组左前扣带回mI/Cr高于右前扣带回,差异有统计学意义(P=0.000);AD组不但左、右前扣带回mI/Cr有差异性(P=0.000),而且左、右后扣带回mI/Cr有差异性(P=0.001).结论 多体素1H-MRS可以检测AD患者左右扣带回代谢物非对称性变化,对AD早期诊断有一定的意义.
Objective To evaluate the characteristics of asymmetry metabolism about cingulate gyrus in Alzheimer’s patients by multivoxel 1H-MRS.Methods Twenty cases of Alzheimer's disease(AD),sixteen patients with mild cognitive impairment(MCI)and seventeen normal control(NC)were detected with multivoxel 1H-MRS for measuring the(N-acetylaspartate,NAA)/(myo-inositol,mI),NAA/(Creatine,Cr),(Cholines,Cho)/Cr and mI/Cr ratios in the posterior cingulate gyrus(PCG)and anterior cingulate gyrus(ACG)bilaterally.The MRS data were analyzed using the statistical software SPSS 17.0.The left-right asymmetry of MRS data were also tested by paired t-test in PCG and ACG.Results In NC,the ratio value of metabolites NAA/Cr,mI/Cr,Cho/Cr and NAA/mI did not show any significant differences between left and right ACG or PCG.In MCI,the mI/Cr showed significant differences between left and right ACG(P=0.000).In AD,the mI/Cr showed significant differences not only between left and right ACG(P=0.000),but also between left and right PCG(P=0.001).Conclusion The multivoxel 1H-MRS can detect the asymmetry metabolism about cingulate gyrus in AD,is useful for early diagnosis of the disease.
