外周血Treg细胞亚群及其细胞因子与心肺功能在COPD稳定期患者中的相关性研究 被引量:3

摘要 目的 观察Treg亚群细胞比例及其相关细胞因子在COPD稳定期患者外周血中的表达情况,分析其与心肺运动功能的相关性.方法 选取2016年1月至2017年12月期间COPD稳定期患者30例作为观察组,体检中心健康人员30例作为正常组,分别进行心肺运动功能试验(CPET)及肺功能测定.采用流式细胞学检测研究对象外周血Treg细胞亚群比例,ELISA方法检测L-17及TGF-β的表达水平.比较研究对象外周血Treg细胞亚群比例、IL-17和TGF-β、心肺运动功能及肺功能变化情况.结果 观察组Treg细胞亚群表达比例较正常组降低(P<0.05),TGF-β及IL-17表达水平升高(P<0.05);观察组患者用力肺活量(FVC)、第1秒用力呼气容积与用力肺活量比值(FEV1/FVC)较正常组降低(P<0.05);峰值公斤摄氧量(Peak VO2/kg)、峰值功率(Peak Power)、峰值CO2排出量(Peak VCO2)较正常组降低,且上述指标与Treg细胞亚群比例呈正相关,与IL-17及TGF-β表达水平呈负相关(P<0.05).结论 COPD患者运动耐量减低,咳嗽、咳痰、呼吸困难或气喘等临床症状的反复发生与Treg细胞亚群比例及其相关细胞因子的变化相关. Objective To observe the proportion of Treg cells and the expression of related cytokines in peripheral blood of COPD patients,and to analyze their relationship between cardiopulmonary motor function.Methods 30 healthy persons in physical examination were selected as normal group.30 cases of inpatients with stable COPD were selected in the experimental group.CPET and lung function test were performed respectively.Flow cytometry was used to detect the proportion of Treg cells in peripheral blood,and the expression level of L-17 and TGF-βby ELISA method.The ratio of Treg cells in peripheral blood,IL-17 and TGF-β,cardiopulmonary exercise and pulmonary function changes were compared.Results The expression of TGF-βand IL-17 was increased in the observation group(P<0.05).The peak VO2/kg,peak power and FVC,FEV1/FVC in the observation group were lower than those in the normal group(P<0.05),and there was a positive correlation between the above indexes and the proportion of Treg cells(P<0.05).There was a negative correlation between IL-17 and TGF-βexpression(P<0.05).Conclusion The recurrent occurrence of clinical symptoms is related to the proportion of Treg cells and the changes of related cytokines in COPD patients.
