Average Hospitalization Day Management based on DRGs System

目的 探讨基于DRGs系统科学管理平均住院日的长效机制.方法 运用DRGs工具根据分析结果 产出的CMI值结合2017年7月-12月实际住院日对临床科室原有平均住院日定额进行调整,对2017年1月-7月与2018年1月-7月的63252例出院病案首页进行分析比较.结果CMI值与平均住院日呈正相关关系.依据CMI值调整住院日定额后,其相关性由0.383提升至0.609,2组相关数据均有统计学差异,全院平均住院日从调整前的10.5天下降到9.4天,秩和检验显示具有统计学差异.结论 基于DRGs系统的平均住院日管理是行之有效的改进方法.

Objective To explore the long-term mechanism of the average hospitalization day based on the scientific management of DRGs.Methods The DRGs tool was used to analyze the 63,252 cases of discharged medical records from January to July 2017 and from January to July 2018.The CMI value based on the analysis results was combined with the actual average hospitalization days from July to December 2017.The hospitalization day quota was adjusted.Results There was a positive correlation between CMI and mean hospitalization days.After adjusting the hospitalization quota according to the CMI value,the correlation increased from 0.383 to 0.609.The data of the two groups were statistically significant.The average hospitalization day of the hospital decreased from 10.5 days before adjustment to 9.4 days.The T test was statistically significant.Conclusions The average hospitalization day management based on the DRGs system was effective.
