老年2型糖尿病不同胰岛素治疗方案的效果评价

Evaluation on Effect of Different Insulin Treatment Regimens in the Elderly with Type 2 Diabetes Mellitus

目的:评价采用不同胰岛素治疗方案治疗老年2型糖尿病的效果。方法:选取2015年3月—2016年7月我院收治的260例老年2型糖尿病患者,按随机数字表法分为A组和B组,各130例。A组采用胰岛素泵治疗,B组采用胰岛素注射治疗。比较两组治疗前后血糖指标变化、低血糖发生率,统计随访期间(1年)的并发症发生率。结果:治疗后两组空腹血糖(FBP)、餐后2 h血糖(2 h PB)和糖化血红蛋白(GHb)与治疗前比较均降低,且A组低于B组,差异有统计学意义(P <0.05);A组低血糖发生率、肾病、视网膜病变、血管病变发生率均低于B组,差异有统计学意义(P <0.05)。结论:与注射方式比较,胰岛素泵治疗老年2型糖尿病更能有效降低FBP、2 hPB和GHb的水平,减少低血糖及并发症发生率。

Objective:To evaluate the effect of different insulin treatment regimens in the treatment of type 2 diabetes mellitus in the elderly.Methods:260 elderly patients with type 2 diabetes mellitus admitted to our hospital from March 2015 to July 2016 were selected and randomly divided into group A and group B,130 cases each.The patients in the group A were treated with insulin pump,and the patients in the group B were treated with insulin injection.The changes in blood glucose indexes and the incidence of hypoglycemia before and after treatment were compared between the two groups,and the incidence of complications during the follow-up period(1 year)was statistically analyzed.Results:The levels of fasting blood glucose(FBP),postprandial 2 h PB(2 h PB)and glycosylated hemoglobin(GHb)after treatment were lower than those before treatment,and the levels in the group A were lower than those in the group B,with statistically significant differences(P<0.05).The incidences of hypoglycemia,nephropathy,retinopathy and angiopathy in the group A were lower than those in the group B,with statistically significant differences(P<0.05).Conclusion:Compared with the injection method,insulin pump can effectively reduce the levels of FBP,2 hPB and GHb,and reduce the incidences of hypoglycemia and complications in the elderly with type 2 diabetes mellitus.
