我院门诊不合格处方分析与改进措施

Analysis and Improvement Measures for Unqualified Prescriptions from Outpatient Clinics in our Hospital

目的:分析我院门诊不合格处方情况,以此来寻找针对性的改进措施.方法:对我院2017年1月—2017年12月开具的5962张处方进行随机抽查,调查不合格处方情况,并对处方不合格原因进行分析,进而制定针对性的改进措施.结果:5962张处方中,共有不合格处方271张,处方不合格率为4.55%,超药物说明书、药物配伍禁忌、给药方法不当、用药剂量不当、临床诊断与治疗不符、重复用药、处方类型错误、处方填写项目不全等均为门诊处方不合格原因.结论:我院门诊存在不合格处方情况,且不合格情况多种多样,为提高处方合格率,需加强医师处方规范书写意识,对医院门诊处方加强管理与监督.

Objective:To analyze unqualified prescriptions from outpatient clinics in our hospital in order to find targeted improvement measures.Methods:5962 prescriptions issued by our hospital from January 2017 to December 2017 were randomly sampled,and the unqualified prescriptions were investigated.The causes for the unqualified prescriptions were analyzed,and the corresponding improvement measures were formulated.Results:Among the 5,692 prescriptions,there were 271 unqualified prescriptions,and the prescription disqualification rate was 4.55%.The superdrug specifications,drug incompatibility,inappropriate administration methods,inappropriate dosage,inconformity of clinical diagnosis with treatment,repeated drug use,incorrect prescription types,incomplete prescription filling and so on are all causes for the unqualified prescriptions.Conclusion:There are unqualified prescriptions from outpatient clinics in our hospital,and there are various kinds of unqualified conditions.In order to improve the rate of qualified prescriptions,it is necessary to strengthen the consciousness of doctors in normatively writing prescriptions and strengthen the management and supervision of outpatient prescriptions.
