米非司酮联合孕三烯酮对腹腔镜卵巢囊肿摘除术后血清性激素水平的影响

Effect of Mifepristone Combined with Gestrinone on Serum Sex Hormone Levels after Laparoscopic Ovarian Cyst Excision

目的:探讨米非司酮联合孕三烯酮对行腹腔镜卵巢囊肿摘除术患者的临床疗效及术后血清性激素水平的影响.方法:选取2016年3月—2018年2月于我院行腹腔镜卵巢囊肿摘除术的106例患者,按随机数字表法分为两组,各53例.对照组术后1周给予注射用醋酸曲普瑞林(3.75 mg/次,每隔28 d肌注1次,共用3次),观察组术后采用孕三烯酮(2.5 mg/次,2次/周)+米非司酮(10 mg,qd)治疗,比较两组临床疗效、血清性激素水平、不良反应及复发率.结果:观察组治疗总有效率高于对照组,复发率低于对照组,差异有统计学意义(P<0.05);治疗后,两组卵泡刺激素(FSH)、黄体生成素(LH)、雌二醇(E2)及孕酮(P)水平均较治疗前改善,且观察组FSH、LH水平较对照组高,E2、P水平较对照组低,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05).结论:米非司酮联合孕三烯酮用于行腹腔镜卵巢囊肿摘除术患者术后治疗的疗效确切,可有效调节患者血清性激素水平,降低复发率,且安全性较高.

Objective:To investigate the clinical efficacy of mifepristone combined with gestrinone in patients undergoing laparoscopic ovarian cyst excision and its effect on postoperative serum sex hormone levels.Methods:106 patients who underwent laparoscopic ovarian cyst excision in our hospital from March 2016 to February 2018 were selected and randomly divided into two groups,53 cases each.The patients in the control group were given triptorelin acetate for injection(3.75 mg/time,intramuscular injection once every 28 days,3 times)one week after operation,and the patients in the observation group were treated with gestrinone(2.5 mg/time,2 times/week)+mifepristone(10 mg,qd).The clinical efficacy,serum sex hormone levels,adverse reactions and recurrence rate were compared between the two groups.Results:The total effective rate in the observation group was higher than that in the control group,and the recurrence rate was lower than that in the control group,with statistically significant differences(P<0.05).After treatment,the levels of follicle stimulating hormone(FSH),luteinizing hormone(LH),estradiol(E2)and progesterone(P)were improved compared with those before treatment,and the levels of FSH and LH in the observation group were higher than those in the control group,while the levels of E2 and P were lower than those in the control group,with statistically significant differences(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Mifepristone combined with gestrinone is effective in the patients undergoing laparoscopic ovarian cyst excision,which can effectively regulate serum sex hormone levels and reduce recurrence rate,and has higher safety.
