盐酸川芎嗪对慢性肝炎肝纤维化患者血清TIMP-1及瘦素水平的影响

Effect of Ligustrazine Hydrochloride on Serum TIMP-1 and Leptin Levels in Patients with Chronic Hepatitis and Hepatic Fibrosis

目的:探讨采用盐酸川芎嗪治疗慢性肝炎纤维化对患者血清金属蛋白酶组织抑制因子(tissue inhibitor of metalloproteinase,TIMP)-1及瘦素水平的影响.方法:选取2016年3月—2018年5月我院收治的慢性肝炎肝纤维化患者100例,按随机数字表法分为对照组和观察组,各50例.对照组采用多烯磷脂酰胆碱(10 mL+5% 葡萄糖注射液250 mL,qd)治疗,观察组在对照组基础上加用盐酸川芎嗪(120 mg+5%葡萄糖注射液250 mL,qd)治疗,比较两组血清TIMP-1水平、瘦素水平、肝纤维化指标及不良反应.结果:治疗后观察组TIMP-1及瘦素水平均低于对照组,差异有统计学意义(P<0.05);观察组治疗后Ⅳ型胶原(collagen typeⅣ,Ⅳ-C)、层黏连蛋白(laminin,LN)、透明质酸(hyaluronic acid,HA)及Ⅲ型前胶原(procollagenⅢ,PCⅢ)水平均较对照组低,差异有统计学意义(P<0.05);两组治疗期间均未出现明显不良反应.结论:采用盐酸川芎嗪治疗慢性肝炎肝纤维化的效果显著,可有效降低患者TIMP-1及瘦素水平,且安全性好.

Objective:To investigate the effect of ligustrazine hydrochloride on the serum levels of tissue inhibitor of metalloproteinase(TIMP)-1 and leptin in patients with chronic hepatitis and hepatic fibrosis.Methods:100 patients with chronic hepatitis and hepatic fibrosis admitted to our hospital from March 2016 to May 2018 were selected and randomly divided into a control group and an observation group,50 cases each.The patients in the control group were treated with polyene phosphatidylcholine(10 mL+5%glucose injection 250 mL,qd),and the patients in the observation group were further treated with ligustrazine hydrochloride(120 mg+5%glucose injection 250 mL,qd).The serum levels of TIMP-1 and leptin,liver fibrosis indexes and adverse reactions were compared between the two groups.Results:The levels of TIMP-1 and leptin in the observation group were lower than those in the control group after treatment(P<0.05),while the levels ofⅣtype collagen(IV-C),laminin(LN),hyaluronic acid(HA)and procollagenⅢ(PCⅢ)in the observation group were lower than those in the control group after treatment,and the differences were statistically significant(P<0.05).There were no significant adverse reactions during the treatment in the two groups.Conclusion:Ligustrazine hydrochloride has a significant therapeutic effect on chronic hepatitis and hepatic fibrosis,which can effectively reduce TIMP-1 and leptin levels in the patients,and has good safety.
