奎硫平联合乌灵胶囊治疗精神分裂症阴性症状的疗效及对患者D-二聚体、Hcy的影响

Efficacy of Quetiapine Combined with Wuling Capsules in the Treatment of Negative Symptoms of Schizophrenia and Its Effect on D-dimer and Hcy in Patients

目的:探讨奎硫平联合乌灵胶囊治疗精神分裂症的临床疗效以及对患者阴性症状、D-二聚体(D-D)和同型半胱氨酸(Hcy)的影响.方法:选取2016年1月—2017年12月我院收治的精神分裂症患者86例,按随机数字表法分为对照组和治疗组,各43例,对照组口服奎硫平(起始剂量100 mg/d),在此基础上,治疗组联用乌灵胶囊(0.99 g,tid),疗程为8周,比较两组临床疗效、不良反应、治疗前后阴性症状(包括情感迟钝、情绪退缩、情感交流障碍、社交退缩、抽象思维困难、交流缺乏自发性及流畅性、刻板思维)以及D-D和Hcy水平变化.结果:治疗组临床总有效率为93.02%,高于对照组的86.05%,差异有统计学意义(P<0.05);治疗后两组PANSS总分、阳性症状及阴性症状均低于治疗前,且治疗组低于对照组,差异有统计学意义(P<0.05);治疗组5项阴性症状得分均低于对照组,差异有统计学意义(P<0.05);两组治疗后D-D及Hcy水平均低于治疗前,且治疗组低于对照组,差异有统计学意义(P<0.05).结论:乌灵胶囊联合奎硫平治疗精神分裂症的疗效显著,可改善患者阴性症状,降低D-D及Hcy水平,提高患者认知和社交能力,且安全性高.

Objective:To explore the clinical efficacy of quetiapine combined with wuling capsules in the treatment of schizophrenia and its effect on negative symptoms,D-dimer(D-D)and homocysteine(Hcy).Methods:86 patients with schizophrenia admitted to our hospital from January 2016 to December 2017 were selected and randomly divided into a control group and a treatment group,43 cases each.The patients in the control group received oral quetiapine(initial dose:100 mg/d).On this basis,the patients in the treatment group were further treated with wuling capsules(0.99 g,tid).The treatment course was 8 weeks.The clinical efficacy,adverse reactions and negative symptoms(including emotional blunting,emotional withdrawal,emotional communication disorders,social withdrawal,difficulties in abstract thinking,lack of spontaneity and fluency in communication,stereotyped thinking)and changes in DD and Hcy levels were compared between the two groups.Results:The total clinical effective rate in the treatment group was 93.02%,which was higher than 86.05%in the control group,and the difference was statistically significant(P<0.05).The total scores of PANSS,positive symptoms and negative symptoms in the two groups after treatment were lower than those before treatment,and they in the treatment group were lower than those in the control group,with statistically significant differences(P<0.05).The scores of five negative symptoms in the treatment group were lower than those in the control group,and the difference was statistically significant(P<0.05).The levels of D-D and Hcy after treatment were lower than those before treatment in the two groups,and the levels in the treatment group were lower than those in the control group,with statistically significant differences(P<0.05).Conclusion:Wuling capsules combined with quetiapine have a significant therapeutic effect on schizophrenia,which can improve patients’negative symptoms,reduce D-D and Hcy levels and improve patients’cognitive and social abilities,and have high safety.
