埃索美拉唑与奥美拉唑四联疗法对Hp阳性消化性溃疡患者胃酸分泌及胃肠激素的影响 被引量:9

摘要 目的:比较埃索美拉唑与奥美拉唑四联疗法治疗幽门螺杆菌(Hp)阳性消化性溃疡的临床效果.方法:回顾性分析2017年5月—2019年2月我院收治的112例Hp阳性消化性溃疡患者的病例资料,根据治疗方式的不同分为两组,各56例.观察组与对照组分别采用以埃索美拉唑、奥美拉唑为主的四联疗法治疗,联合药物为胶体果胶铋、克拉霉素及阿莫西林,比较两组胃酸分泌情况及胃肠激素水平.结果:治疗后两组高峰胃酸分泌量(PAO)、基础胃酸分泌量(BAO)、最大胃酸分泌量(MAO)较治疗前均降低,且观察组低于对照组,差异有统计学意义(P<0.05);治疗后两组胃泌素(GAS)、胃动素(MTL)水平较治疗前均降低,生长抑素(SS)、降钙素基因相关肽(CGRP)较治疗前均升高,且观察组变化幅度大于对照组,差异有统计学意义(P<0.05).结论:与奥美拉唑四联疗法比较,以埃索美拉唑为主的四联疗法可更好的抑制Hp阳性消化性溃疡患者的胃酸分泌,更好的调控胃肠激素水平,临床治疗效果显著. Objective:To compare the clinical effect of esomeprazole and omeprazole in the treatment of Helicobacter pylori(Hp)-positive peptic ulcers.Methods:112 cases of patients with Hp-positive peptic ulcers admitted to our hospital from May 2017 to February 2019 were retrospectively analyzed,and the patients were divided into two groups according to different treatment methods,56 cases each.The patients in the observation group and the control group were treated with esomeprazole and omeprazole-based quadruple therapy,respectively.The combined drugs were colloidal bismuth pectin,clarithromycin and amoxicillin.The secretion of gastric acid and the levels of gastrointestinal hormones were compared between the two groups.Results:The peak gastric acid output(PAO),basic gastric acid output(BAO)and maximum gastric acid output(MAO)after treatment were lower than those before treatment in the two groups,and the PAO,BAO and MAO in the observation group were lower than those in the control group,with statistically significant differences(P<0.05).The levels of gastrin(GAS)and motilin(MTL)after treatment were lower than those before treatment in the two groups,while the levels of somatostatin(SS)and calcitonin gene-related peptide(CGRP)after treatment were higher than those before treatment,and the changes in the observation group were greater than those in the control group,with statistically significant differences(P<0.05).Conclusion:Compared with omeprazole quadruple therapy,esomeprazole-based quadruple therapy can better inhibit gastric acid secretion in the patients with Hp-positive peptic ulcers and better regulate gastrointestinal hormone levels,and has a significant clinical therapeutic effect.
