丁苯酞软胶囊对缺血性脑血管病患者颈动脉易损斑块稳定性的影响

Effect of Butylphthalide Soft Capsules on Stability of Carotid Vulnerable Plaques in Patients with Ischemic Cerebrovascular Diseases

目的:探讨丁苯酞软胶囊对缺血性脑血管病患者颈动脉易损斑块稳定性的影响.方法:选取2016年3月—2018年3月我院收治的缺血性脑血管病患者96例,且通过颈动脉超声检查结果均为颈动脉易损斑块,按照随机数字表法分成对照组与观察组,各48例,两组均给予常规治疗,观察组加用丁苯酞软胶囊(0.2 g,tid),比较两组炎性因子、斑块稳定性.结果:治疗后观察组基质金属蛋白酶(MMP)-9、超敏C反应蛋白(hs-CRP)、细胞间黏附因子(ICAM)-1、血清核转录因子 κB(NF-κB)水平及颈动脉内-中膜厚度值(IMT)、斑块Crouse积分均低于对照组,差异有统计学意义(P<0.05);观察组不稳定斑块检出率为32.98%,低于对照组的65.63%,差异有统计学意义(P<0.05).结论:丁苯酞软胶囊可有效降低缺血性脑血管病患者的炎性因子水平,提高颈动脉易损斑块的稳定性,利于预后.

Objective:To investigate the effect of butylphthalide soft capsules on the stability of carotid vulnerable plaques in patients with ischemic cerebrovascular diseases.Methods:96 patients with ischemic cerebrovascular diseases admitted to our hospital from March 2016 to March 2018 were selected,and the results of carotid ultrasonography were all carotid vulnerable plaques.The patients were randomly divided into a control group and an observation group,48 cases each.The patients in the two groups were given conventional treatment,and the patients in the observation group were treated with butylphthalide soft capsules(0.2 g,tid).The inflammatory factors and plaque stability were compared between the two groups.Results:After treatment,the levels of matrix metalloproteinase(MMP)-9,hypersensitive C-reactive protein(hs-CRP),intercellular adhesion factor(ICAM)-1,serum nuclear transcription factorκB(NF-κB)and carotid intima-media thickness(IMT)and plaque Crouse scores in the observation were lower than those in the control group,and the differences were statistically significant(P<0.05).The detection rate of unstable plaques in the observation group was 32.98%,which was lower than 65.63%in the control group,and the difference was statistically significant(P<0.05).Conclusion:Butylphthalide soft capsules can effectively reduce the levels of inflammatory factors in the patients with ischemic cerebrovascular diseases and improve the stability of carotid vulnerable plaques,and are conducive to prognosis.
