克罗米芬联合二甲双胍对多囊卵巢综合征患者卵巢形态及血流动力学的影响

Effect of Clomiphene Combined with Metformin on Ovarian Morphology and Hemodynamics in Patients with Polycystic Ovary Syndromes

目的:观察克罗米芬、二甲双胍联合治疗多囊卵巢综合征(PCOS)对患者卵巢形态及血流动力学的影响.方法:选择2017年10月—2018年9月我院收治的PCOS患者112例,应用随机数字表法分为两组,各56例.对照组接受克罗米芬(50 mg,qd)治疗,观察组则在此基础上加用二甲双胍(500 mg,tid)治疗.观察两组临床疗效、卵巢形态指标、血流动力学指标[收缩期最大血流速度(PSV)、阻力指数(RI)].结果:观察组总有效率较对照组高,差异有统计学意义(P<0.05);治疗前两组卵巢体积、卵巢包膜厚度、卵泡最大直径、PSV、RI水平比较,差异无统计学意义(P>0.05);治疗后观察组卵巢体积、卵巢包膜厚度、RI水平低于对照组,卵泡最大直径、PSV水平高于对照组,差异有统计学意义(P<0.05).结论:克罗米芬联合二甲双胍治疗PCOS的疗效显著,可改善患者的卵巢形态,调节血流动力学.

Objective:To observe the effect of combined treatment with clomiphene and metformin on ovarian morphology and hemodynamics in patients with polycystic ovary syndromes(PCOS).Methods:112 patients with PCOS admitted to our hospital from October 2017 to September 2018 were selected and randomly divided into two groups,56 cases each.The patients in the control group were treated with clomiphene(50 mg,qd),and the patients in the observation group were treated with metformin(500 mg,tid).The clinical efficacy,ovarian morphology and hemodynamic indexes[systolic maximum blood flow velocity(PSV),resistance index(RI)]were observed.Results:The total effective rate in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).There were no statistically significant differences in ovarian volume,ovarian capsule thickness,maximum follicle diameter,PSV and RI levels before treatment between the two groups(P>0.05).After treatment,the ovarian volume,ovarian capsule thickness and RI level in the observation group were lower than those in the control group,while the maximum follicle diameter and PSV level were higher than those in the control group,with statistically significant differences(P<0.05).Conclusion:Clomiphene combined with metformin is effective in the treatment of PCOS,which can improve ovarian morphology and regulate hemodynamics.
