静脉血栓患者的药学监护

Pharmaceutical Care of Patients with Venous Thrombosis

目的探讨临床药师在围生期静脉血栓患者防治过程中的药学监护切入点.方法回顾性分析临床药师参与的1例胎膜早破并发下肢静脉血栓患者的抗凝治疗过程.结果临床药师分析了患者发生静脉血栓的原因,并从患者分娩前后抗凝药物的选择、给药方法、用药疗程等方面为医生提供合理化用药建议并被采纳,取得了很好的疗效.结论临床药师对围生期患者开展药学监护,对避免不良事件的发生及促进临床合理用药有积极的意义.

Objective To explore the entry point of pharmaceutical care for clinical pharmacists in the prevention and treatment of perinatal venous thrombosis patients.Methods A retrospective analysis was made on the anticoagulant treatment of one case of premature rupture of membranes complicated with venous thrombosis of lower extremities.RESULTS Clinical pharmacists analyzed the causes of venous thrombosis in patients,and provided rational drug use suggestions for doctors from the aspects of anticoagulant drug selection,drug delivery methods and drug treatment course before and after delivery,and achieved good results.Conclusion Clinical pharmacists'pharmaceutical care for perinatal patients is of positive significance in avoiding adverse events and promoting rational drug use in clinic.
