双气囊小肠镜经肛进镜单人法配合枳实消痞汤的临床应用

Clinical Application of double-balloon Endoscopic single-person method combined with Zhishi Xiaopi decoction through anal Endoscope

目的: 探讨经肛双气囊小肠镜快速抵达空肠安全、有效的插入方法, 以及中药对小肠镜术后不良反应的治疗效果.方法: 选取行双气囊小肠镜检查的84 例患者, 随机分为对照组、观察组各42例.对照组采用双气囊小肠镜双人插入法, 术后服用枸橼酸莫沙必利分散片组;治疗组采用双气囊小肠镜单人插入法, 术后服用枳实消痞汤.比较两种方法下内镜在相同时间内插入深度、贯通率, 术后不良反应的缓解时间、术后肠鸣音的恢复时间.结果: 两种方法成功率均为100% , 治疗组在相同时间内的插入深度及贯通率, 以及术后不良反应缓解时间、肠鸣音恢复时间、治疗效果均明显优于对照组 (P <0. 05) .结论: 双气囊小肠镜单人插入法是一种可行的、安全、更有效快捷的抵达空肠甚至贯通的进镜方法, 枳实消痞汤能有效地缓解小肠镜术后不良反应, 值得临床推广应用.

Objective To discuss the safe and effective insertion method of fast access to jejunum by double balloon enteroscopy through anus and the therapeutic effect of traditional Chinese medicine on adverse reactions after small bowel surgery.Methods Eighty-four patients underwent double balloon enteroscopy were randomly divided into control group(n=42)and observation group(n=42).The patients in the control group were treated with double balloon enteroscope double insertion and mosapride citrate dispersible tablets after operation,and the patients in the treatment group were treated with double balloon enteroscope single insertion and Zhishi Xiaopi decoction after operation.The insertion depth,penetration rate,remission time of postoperative adverse reaction and recovery time of postoperative intestinal sound were compared between the two methods in the same time.Results The success rate of the two methods was 100%.The insertion depth and penetration rate of the treatment group were significantly better than those of the control group(P<0.05),as well as the postoperative relief time of adverse reactions and the recovery time of intestinal sound(P<0.05).Conclusion Double-balloon colonoscopy single insertion method is a feasible,safe,more effective and efficient approach to reach the jejunum and even through the endoscope.Zhishi Xiaopi decoction can effectively alleviate the adverse reactions after small bowel surgery,and is worthy of clinical popularization and application.
