膀胱内翻性乳头状瘤的诊治(附12例报告并文献复习)

Diagnosis and Treatment of Inverted Papilloma of the Bladder(Report of 12 Cases and Literature Review)

目的:探讨膀胱内翻性乳头瘤的诊断与治疗。方法回顾性分析12例膀胱内翻性乳头瘤的临床资料。10例采用经尿道膀胱肿瘤电切术(TURBt),1例行膀胱部分切除术,1例输尿管镜下钬激光切除。术后膀胱灌注化疗预防复发。结果12例获得随访1年-5年,1例肿瘤复发,未见恶变。结论膀胱内翻性乳头瘤是一种少见的尿路上皮肿瘤,预后良好。诊断依赖于膀胱镜检查及术后病理结果,B 超和 CT 检查可作为筛查的手段,TURBt 是其标准的治疗方法,术后膀胱灌注化疗有利于预防复发。

Objective To study the diagnosis and treatment of inverted papilloma of the bladder(IPB).Methods A retrospective study was done including reviewing the clinical documents of 12 cases of IPB.10 cases were treated with transurethral resection of bladder tumor(TURBt).1 cases partial cystectomy and 1 case cutted by holmium laser under ureteroscope.All patients were treated with imigation of bladder with chemotherapeutic drugs after the operation to prevent recurence.Results All cases were followed u about 1 to 5 years,I case recurence,without canceration.Conclusion Inverted papilloma of the bladder is rare urothelial tumor and prognosis is good.Dignosis methods depend on cystoscopy and pathobiology after operation.B-mode ultrasonography and CT are the important screening method.TURBt is the standard therapeutic choice.Bladder rrigation therapy after operation is good to prevent the tumor recurrence.
